I hereby release, waive, and hold harmless WTX Sports Academy, its owners, coaches, staff, and volunteers from any and all claims, liabilities, or damages arising from my child’s participation, except in cases of gross negligence.

I certify that my child is physically able to participate and that I assume full responsibility for any injury or medical expenses that may occur. In the event of an emergency, I authorize WTX Sports Academy staff to obtain medical treatment for my child.
