World Between Pages – Parent/Guardian Consent & Waiver
I, the undersigned parent or legal guardian of the participant named above, give permission for my child to participate in the World Between Pages reading and imagination event (“WBP”).
I understand that this program may include light physical movement, sensory activities, and group interaction. I acknowledge that participation carries some inherent risks, including the possibility of minor injury or illness, and I accept these risks.
On behalf of myself, my child, and our heirs, executors, and assigns, I release and hold harmless the organizers of WBP, including volunteers, sponsors, agents, and employees, from any liability for injury, loss, or damage to person or property that may arise from my child’s participation in WBP, whether caused by negligence or otherwise.
I further agree to indemnify and hold harmless WBP and St Philip AME Church against any claims or costs resulting from my child’s participation.
I certify that my child is physically able to participate and has no medical conditions that would prevent safe involvement in the event.
I also grant permission for emergency medical treatment to be administered to my child if necessary.
Child Release Policy:
For the safety of all participants, your child will only be released to individuals listed on the Authorized Pick‑Up list.
Staff will request photo identification from anyone picking up a child.
Children will not be released to anyone who is not on the authorized list, without exception.
Any changes to the pick‑up list must be submitted in writing by the parent/guardian.
Photo Release Consent
I give permission for photos and videos taken of my child during the WBP Reading Event to be used by the organizers for promotional, educational, and marketing purposes, including on social media, websites, and printed materials. I understand that my child’s name will not be used without additional consent.
