I Parents Name * am registering my child Child's Name * for the 2026 Winter basketball season. I understand that there is a season fee associated with their participation. I understand that the entire season fee is due, payable to D1 Prospects/Texas Prime, even if my child gets injured or decides for whatever reason they do not want to finish the season. By signing this agreement, I am also attesting that I have medical coverage for my child and that my child is healthy enough to participate in the intense and vigorous workouts of the program. I will not hold D1 Prospects, Texas Prime Athletics, any Coaches associated with D1 Prospects or Texas Prime Athletics, or the facilities and staff of the facilities that we will practice and play games in, responsible for any injuries my child may get while participating in the program
