  • Please provide us with a copy of all pages of all tax forms received from each investment company. Each page of a tax form may contain information important to the preparation of your return.

  • Please provide us with a copy of the settlement sheet from the sale, refinancing, and/or home equity loan along with your other tax information. We will also need the same documentation from the initial acquisition of the property.

     

    If HELOC/Cash-Out Refinannce funds were used to buy, build, or improve the home, please provide us with specifics and/or receipts.

  • Please provide details regarding each energy efficient property added to your property along with your other income tax information.

  • Please provide a copy of the sales invoice containing the sales tax paid.

  • Retirement Plans

  • Please provide the name(s) and amount(s) that were directly transferred from your retirement account(s) to each charity so that the taxable amount of your retirement distribution can be reduced accordingly. Please be certain to exclude any charitable contributions made from your retirement account(s) from the details of any charitable contributions made since they cannot be counted twice.

  • Health Care Premium Assistance

  • If you purchased your health insurance through the health insurance marketplace (healthcare.gov), please provide a copy of Form(s) 1095-A (Health Insurance Marketplace Statement).

  • Education

  • Please include the letter received from the Louisiana START program in January along with your other tax information so that the Louisiana income tax deduction can be claimed. Be certain to list the name of each designated beneficiary alongside their account number and annual contribution on the back of the letter received. Each contribution must be accompanied by the designated beneficiary's name to claim the deduction.

  • Please include a copy of the Form 1099-Q issued reporting the distribution(s) with your tax materials.

  • Please be sure to include details (separately by student) for these expenses along with a copy of any Forms 1098-T received from the educational institution(s) with your other income tax information.

  • Please include a copy of the Form 5498-SA provided to you by your HSA administrator with your tax materials. This form reports the HSA contributions made during the year to the Internal Revenue Service.

  • Please include a copy of the 2025 Form 1099-SA provided to you by your HSA administrator with your tax materials. This form reports the HSA distributions made during the year to the Internal Revenue Service.

  • Please provide the name, address, and Social Security Number of each person and the amount paid to him/her along with your other income tax information.

  • If you received any such correspondence, please include a copy with your other income tax information.

  • Please provide the IRS notice which assigned you and your spouse (if applicable) PIN numbers. If assigned, the PIN number is required to electronically file an income tax return with the IRS.
