The iResearch Institute applicant and parent (s)/guardian(s) must read and sign the following agreement as a condition of the applicant’s participation in the iResearch Institute Summer Research Program. I (We), the undersigned, certify that I am the parent or legal guardian of First Name*   Last Name*  and that I have the legal authority to make decisions for this student applicant. We have reviewed the iResearchInstitute.com website for the description of the summer science research program for which the student applicant is applying. We fully understand that the iResearch Institute (DBA under the iResearch Corporation) does not guarantee success or participation at local, regional, national, or international competitions or positive outcomes resulting from experimentation and participation in the summer program.

Student participants should work to achieve educational goals with the support of the educators of the iResearch Institute. If accepted to the program, we and the student must follow all rules and guidelines for student conduct, including all directions regarding preparation, review of lessons, contact between other members of the iResearch Institute cohort, and plagiarism including inappropriate use of AI. We (parent/guardian and student) realize that the iResearch Institute reserves the right to require the student to leave the program for medical, disciplinary, or other reasons, at its sole discretion. If a student is removed from the iResearch Corporation programs, we understand that the student must depart the program immediately; costs or associated losses of participation fees are solely the responsibility of the parent/guardian and student. If the student is asked to leave the iResearch Institute for disciplinary reasons (i.e., harassment or otherwise), we understand that tuition will not be refunded and that the student may not be allowed to attend future summer institutes. We understand that under extenuating circumstances, it may be necessary for summer science research program staff to intervene for the safety of the students and others participating in the iResearch Institute.

Students and parents/guardians also understand that if accepted to the iResearch Institute additional waivers and agreements may be required as a prerequisite of being participating in the iResearch Institute.

Parent/Guardian:
I understand that:
- Application fees will be processed immediately upon receipt of my application regardless of acceptance and accepted student's tuition payments will be processed immediately upon receipt.
- Tuition fees paid will only be refunded as outlined on the iResearch Institute website. Refunds will be issued only under the following circumstances: if a student withdraws for a medical reason and provides appropriate documentation from a physician or the student did not receive financial aid; refunds will be given on a prorated basis as noted - https://www.iresearchinstitute.com/application-info
- Tuition payment(s) are my responsibility, paid per the due date on the invoice.
- The application fee will NOT be refunded under any circumstances.
- I am responsible for any incidental expenses not covered by the tuition (i.e. lunch, field trips, etc.).
- I am responsible for any medical costs incurred by my child while enrolled in the iResearch Institute.

I permit for:
- My child participates in all online/virtual activities associated with iResearch Institute student success.
- My child to be videotaped, photographed, and interviewed for broadcast or publication, and/or have a sample of his or her work broadcast or published. I understand that the iResearch Institute (DBA under the iResearch Corporation) will exercise discretion regarding media contact.

By signing your name(s) in the spaces below, you indicate that you and the student have read and agree to the terms of the Student and Parent Agreement.

I, the undersigned, certify that I am the parent or legal guardian of   First Name*   Last Name*   and that I have the legal authority to make decisions for this student applicant.

I understand science research experimentation and participation in the iResearch Institute poses potential risks and I agree to accept any and all risks associated with the above-named student applicant’s participation in the iResearch Institute. I am voluntarily signing the student applicant up for the iResearch Institute (DBA under the iResearch Corporation), and recognize and acknowledge that physical injury, accident, illness, death, loss of personal property, or other contingencies may befall upon participants while involved in the iResearch Institute summer research program. I agree to release and indemnify, defend, and hold harmless, iResearch Corporation employees and directors, shareholders, members, mentors, fellows, and chaperones of the iResearch Institute and its officers, employees, and agents from any and all liability and damages or losses I or my child (minor) may suffer to our persons or property or both which arise out of or occur during my child’s participation in the iResearch Institute summer research program except if the damages or losses are caused by the gross negligence or willful misconduct of iResearch Institute employees. I also agree to indemnify and hold harmless the iResearch Corporation and its officers, employees, and agents, from liability for myself or the student applicant’s negligent acts and/or omissions and for losses of or damages to other persons’ personal property for which I or the student are the cause.

I understand that the iResearch Institute (DBA under the iResearch Corporation) carries no medical insurance of any kind for students. Regardless of my insurance status, I release the iResearch Institute (DBA under the iResearch Corporation) from any responsibility and liability for the student applicant’s injuries, illnesses, medical bills, charges, or similar expenses. I agree that this Release Form is to be construed in accordance with the law of the State of New York and that if any portion of this agreement is held invalid, the remainder of the agreement shall continue in full force and effect. I also agree that the exclusive jurisdiction for any dispute arising from or related to the student applicant’s participation in the iResearch Institute (DBA under the iResearch Corporation) or this agreement shall be state or deferral courts located within the State of New York. I acknowledge that I have with my student applicant read this entire document, and I agree to its terms.
