NHSE OpenSAFELY User Application Form
OpenSAFELY provides two different roles within the Service: OpenSAFELY Full User and OpenSAFELY Collaborator. Please read the descriptions of each role, then select the one most appropriate to your needs as a member of the project team.

OpenSAFELY Full Users have the ability to run code, view unreleased outputs, and request their release.

OpenSAFELY Collaborators can contribute to writing code and reviewing the released results prior to their publication.

GENERAL INFORMATION

Please note that the below will be required:
- If you are applying to be an OpenSAFELY Full User, you must provide a UK phone number, as this is required to access the VPN for the service. This can be a personal or employer-issued mobile phone.
- All OpenSAFELY Full Users who wish to become OpenSAFELY Project Leads are required to work for/study at an organisation with a Data Sharing Framework Contract (DSFC) with NHS England, before your application can be approved. If you are applying for Full User status and your organisation does not have a current DSFC, we will send you details of how to apply.
*We may contact you using the details you have provided to discuss your application or project and to provide you with support. From time to time, we may also seek to confirm that the OpenSAFELY Service is running effectively by means of a short, non-obligatory survey.
