Surgery Transportation Request Form
Transport from the Marysville Cat Resource Center to The Hathaway Clinic in Lincoln, Ca. Cats can be dropped off between 7:30 - 8:00 am on the date of surgery and must be picked up the same day. Pick up times vary so please plan to be availabile for pick up in the evening. Submission of this form doesn't guarantee transport, you will receive a confirmation email if your request is approved. If you do not request transport at least 5 days in advance we can not guarantee transport. Transport is for Mondays and Wednesdays only.
