    Group Registration Form - This form is for leaders who are bringing a group of kids from their church or organization.
  • Contact Information

  • Reserve Your Spots

    • At least one leader from your group must be 25+.  
    • Other Chaperones must be 18+ and at least 2 years older than your oldest camper.
    • For the Tween retreat, you may bring a high school helper or two, but you should count them as "campers" and not as "leaders" when it comes to your ratios.  Also, please alert Renee of any hs helpers you plan to bring as soon as possible.
    • Must have a leader for each gender of students attending. If you only bring girls, then you only need to bring female leaders. 
    • There must be at least an 1:8 ratio of adult leaders to campers. Please match gender of leaders to gender of campers. 
    • Please jot down the number of spots you reserve and post it in your workspace.  If it looks like your numbers are going to go over once registrations start coming in, please email Renee asap to see if more spots can be reserved for your group. 
  • Housing

  • Cottages

    • The cottages are Crestfield's default housing for youth retreats.  
    • Built in 2014 these buildings have three bunk rooms and two dorm style bathrooms (one in each building is handicap accessible).  
    • Most of the beds in the cottages are twins, however there is one queen sized bed in each of the upstairs rooms.
    • The cottages are located a short walk away from main camp, but groups are welcome to drive back and forth to main camp provided the driver is 25 or older.  
    • Participants staying in the cottages are responsible for bringing both their own bedding and their own towels.
    • Housing in the cottages is determined by the sex of the participant.  Boys stay together, usually in Omega Cottage, and girls stay together, usually in Alpha Cottage.  

    Scott Lodge

    • Scott Lodge may be requested by groups who have participants with special considerations such as medical concerns, behavioral issues, or a unique need for increased privacy which cannot be addressed in the cottage/dorm style setting.  
    • Scott Lodge was built at main camp in 1999 and is a hotel style facility in which each room has a private bath. 
    • There are two types of Scott Lodge Rooms. The larger rooms have a queen bed and two twins.  The smaller rooms have two twins. For youth retreats we will also add an additional participant on an extra mattress/cot in each room, so that the larger rooms sleep 5 and the smaller rooms sleep 3.  
    • Bedding will be provided in Scott Lodge Rooms, but towels will not be.  
    • Groups who need to stay in Scott Lodge will be assigned up to 4 rooms free of charge based on their numbers and the needs of their participants.  Groups that are large enough to require more than 4 rooms will be charged $100 for each additional room.  
    • Room assignments in Scott Lodge are determined by the church leaders in conversation with their participants and their participants' parents.  We do ask, however, for Child Protection purposes that leaders assign unrelated minors in groups of no less than 3 per room.  

     

  • Camper Medications

  • Crestfield does not staff a Camp Medic for Retreats.  This means that as the church leader bringing your group to camp, you and your other chapersones are responsible for collecting and administering the medications to the campers in your group.  Our registration form collects standard medical and medication related information from parents and you will recieve a copy of that completed registration form when your campers register.  You need to be aware that the following instructions are given to camper parents when they register their child for our retreats.  

    Instructions and Notes about Medications:

    • Send all medications in original packaging, but only include as many doses as your child will need for the duration of camp.  
    • Put all medications together in a zip lock bag labeled with your child's first and last name and the name of your church.  Inside the bag, include a typed or neatly written note explaining dose times and amounts.  
    • Please give your child's medication bag to your church group leader when you drop them off for camp.  
    • While at camp, your child's medication will be tracked and administered by their church leader. 
    • If your child's medication requires refridgeration or other unique storage, please note that above and communicate it to your church leader as well. 

    Leaders may keep camper medication with them at camp, or they may store the medication in our Program Supply Room (aka Bull Pen) in the camp office.  Please let Renee know if you wish to store it in the Supply Room.  

  • Individual Registrations and Payment

  • Final Numbers/Individual Registrations: 

    • Links to Individual Camper and Chaperone Registration Forms will be emailed to you on or about January 19th.
    • Individual Camper and Chaperone Registrations will be due on Wednesday February 18th. -- If you have unused spots, they will automatically be dropped at this time.
    • If it looks like your numbers are going to go over the number of spots you reserved, please contact Renee asap to see if additional spots can be reserved for your group.  
    • Your payment will be based on the number of individual registrations we recieve from your group. 

     Cost: 

    • $100 each for both Campers and Chaperones.
    • $100 per additional room will be charged to groups that require more than 4 Scott Lodge rooms.  
    • Individuals who register but end up not attending will owe a partial payment of $30 to cover sunk costs.

    Financial Aid:

    • Financial Aid is available for churches that serve under-resourced communities.  

    FAQ:

    • Please make sure to download the Retreat Leader's Guide (but also don't get distracted doing that to the point that you forget to hit "submit" on this form).  In it you will find the steps involved in the registration process, a packing list you can give your participants, and a list of camp expections that you can pass on to your group as well.
