  • Below you will find a series of questions. This form is secure and confidential. Please fill out the information below before the deadline listed on your summons. Please be sure to provide an e-mail address that is valid and that you check regularly. If you are excused from service based on your responses below, or if the jury panel is canceled, you will be notified by e-mail. 

    Right to Reemployment: An employer may not discharge, threaten to discharge, intimidate, or coerce any permanent employee because the
    employee serves as a juror, or for the employee's attendance or scheduled attendance in connection with the service, in any court in the United States.
    An employee who is discharged, threatened with discharge, intimidated, or coerced in violation of this section is entitled to return to the same
    employment that the employee held when summoned for jury service if the employee, as soon as practical after release from jury service, gives the
    employer actual notice that the employee intends to return. (Civil Practice and Remedies Code Section 122.001)

    Failure to Answer Summons and Provision of False Information: Absent a reasonable excuse, a person may be fined not less than $100 nor
    more than $500 if the person fails to attend court as required by this summons. (Article 19A.054, Code of Criminal Procedure)

  • Exemptions, Disqualifications, and Excuses

  • State law does not allow a person to be excused for economic or business reasons. However, if you have hardships or concerns, please list them below and they may be addressed when you appear for Court.

    The law does provide certain other exemptions that you can claim below as well as a list of factors that may disqualify you for jury service.

    If you claim an exemption or disqualification, you are only formally excused when you receive an e-mail from the Court notifying you that you are excused from service. If you do not received a formal e-mail notifying you that you are excused, you may be asked to appear to explain your request for exemption or disqualification.
