I hereby give Jaze Companies (including it's associated brands: Jaze Photography, Jaze Modeling, Pellam Publishing and others) the right and permission to reproduce and or publish photos taken of me on the date(s) specified below for the purpose of building my portfolio and/or promoting me for work with other photographers. I understand these photos of me may be in whole or in part, or composite, or distorted in character or form for the purpose of creative effect.
For portfolio photos, I understand that Jaze Companies holds the primary copyright with myself listed as a joint holder. This joint holding allows me to use the photos to promote myself as I wish permitting that copyright credit is given to myself and Jaze Jaze Companies. Any agreements to purchase or sell said photographs will be negotiated by Jaze Jaze Companies. I grant the right and permission to Jaze Jaze Companies to take sole copyright of any said photographs for the purpose of negotiating a purchase agreement or corporate promotion. I acknowledge that Jaze will notify me in writing with as much advance notice as possible which photographs Jaze will take sole copyright of and I certify that I will make the proper copyright credit changes on any outstanding photographs.
I understand that Jaze Jaze Companies will work with me when making their selections for promotional efforts but that my consultation is not required. I agree that when my consultation is not available I waive my option to inspect and/or approve the finished product or printed matter and will entrust Jaze Jaze Companies to protect my image and to ensure the quality of the finished product.
I hereby release, discharge, and agree to save Jaze Jaze Companies from any liability by virtue of blurring, distortion, alteration, optical illusion, or use in composite form whether intentional or otherwise, that may occur in taking of said pictures or in any processing tending towards the completion of the finished product, or its publication or distribution.
