Liability Waiver
By signing up for the Colin Jacobs 2025 Summer Camp, participants agree to the following terms and conditions:
1. Assumption of Risk: I understand and acknowledge that participation in the Colin Jacobs Hockey Camp 2026 involves inherent risks and hazards, including but not limited to physical injury, property damage, and the risk of contracting illnesses.
2. Waiver and Release: In consideration of being allowed to participate in the Colin Jacobs Hockey Camp, I hereby waive, release, and discharge, Colin Jacobs, and all organizers, instructors, volunteers, and affiliates from any and all liability for any claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may occur during or as a result of my participation in the Hockey Kids Camp.
3. Indemnification: I agree to indemnify and hold harmless, Colin Jacobs, and all organizers, instructors, volunteers, and affiliates from any and all liabilities, claims, demands, actions, or causes of action, including attorneys’ fees and costs, arising out of or related to my participation in the Hockey Kids Camp.
4. Certification of Physical Condition: I certify that I am in good physical condition and have no medical conditions that would prevent me from safely participating in the Hockey Kids Camp. I agree to follow all instructions and guidelines provided by the organizers of the camp.
5. Permission for Use of Likeness: I grant permission for the use of my likeness in photographs, videos, or other media for promotional or educational purposes related to the Hockey Kids Camp.
