I, the parent or legal guardian of the above-named child, understand that participation in Jewel City United tryouts involves physical activity and may carry a risk of injury. I voluntarily allow my child to participate and assume all risks associated with participation.

I hereby release, waive, and hold harmless Jewel City United, its directors, coaches, staff, volunteers, and affiliates from any and all claims, liabilities, or demands arising from my child’s participation in the tryout, except in cases of gross negligence.

I confirm that my child is physically able to participate and that I am responsible for any medical expenses that may result from injury.
