I understand that participation in personal training, semi-private training, strength and conditioning, basketball training, and athletic performance training involves physical activity that may carry inherent risks, including but not limited to muscle soreness, fatigue, dizziness, injury, or other physical strain.
I confirm that I am physically able to participate in exercise and training activities and that I have disclosed any relevant injuries, medical conditions, or limitations prior to beginning training. I understand that it is my responsibility to communicate discomfort, pain, or concerns during any session.
I voluntarily assume full responsibility for any risks, injuries, or damages that may occur as a result of participation. I hereby release, waive, and hold harmless Mark Aquino / Mark Aquino Fitness Coaching from any and all claims, demands, or causes of action arising out of or related to my participation in training sessions.
I understand that this waiver applies to all current and future training sessions unless otherwise stated in writing.
