    This Section Must Be Filled Out By The Parent Or Legal Guardian Applying For Assistance.

  • Eligibility Requirements:

    • Must not have received funds from Project Sweet Peas in the past 12 months
    • Child(ren):
      • Must be in custody of parent applying
      • Must have been in NICU for minimum of 2 weeks
      • Must STILL be under care on date of submission

    You must upload one unedited photo of your ID placed next to your infant(s), with both visible in the same photo taken at the same time.

    Verification Photo Requirement:
    You must upload one unedited photo of your ID placed next to your infant, with both clearly visible in the same photo taken at the same time.

    • This can be taken next to an isolette or crib 
    • Your baby’s face or identifying features may be covered if you prefer
    • This photo is used for verification purposes only and will not be shared

    Important:

    • We cannot accept separate photos, edited images, or combined/photoshopped images
    • If your infant absolutely cannot be included, email foodandfuel@projectsweetpeas.com for alternative verification process information. 

    Please note:
    Applications submitted without the proper verification photo, or with suspected editing or tampering, will be automatically denied.

    Option 2: Card or Gift Card Options
    Choose the format that works best for you:

    • Digital Visa card
    • Digital gift cards for select gas and food retailers
    • Physical Visa card
      • arrives in a plain white envelope within 1–3 weeks

    All funds are distributed through Tremendous.com, and you will make your final selection when redeeming your link.
