Please carefully review the following requirements before submitting your application. This opportunity is ideal for individuals who value elevated care, expert providers, and exceptional outcomes.

MODEL ELIGIBILITY REQUIREMENTS

Age: Applicants must be 21 years or older

Location: Must be able to travel to our Owings Mills, MD location

Availability: Must be available during designated model dates - Mondays/Wednesday/ Selective Saturdays

TREATMENT CRITERIA

Models may be selected to receive treatment in one or more of the following areas:

Dermal Fillers - Full Facial Balancing AVAILABLE 05/01

Laser Skin Resurfacing - Surgical Scars - AVAILABLE May 2026

Botox (Model spots filled - check back in June 2026)

PRP/PRF Dermal Filler (Collagen-Stim)(Model spots filled - check back in May 2026)

Solid Filler (PDO Threads) (Model spots filled - check back in June 2026)

Important Notes:All treatments are performed by licensed medical providers for educational and content creation purposes

Touch-ups are not included unless otherwise noted. As this is a training or marketing session, revisions or follow-up treatments are not guaranteed at the model rate.

PHOTO & VIDEO CONSENT

Selected models must consent to photo and video documentation throughout the treatment process. These assets may be used across:Social media platforms (Instagram, TikTok, Facebook)Website and digital platforms Promotional, marketing, or educational content.

Media Usage Terms:All captured content becomes the exclusive property of 360ENVY Aesthetics & Wellness and may be used indefinitely.

Models must submit clear, unfiltered photos for review, including:

Front and side profiles

No makeup

Neutral expression

Natural lighting with a plain background

ADDITIONAL REQUIREMENTS

Must complete a comprehensive medical intake and consent form prior to treatment

Must not have any active skin infections, open wounds or contraindicated medical conditions at the time of service.

Must maintain responsive communication via text or phone for scheduling and confirmation

Cancellation Policy: We require 48 hours’ notice for any schedule changes. No-shows or late cancellations will result in loss of model eligibility & special rates.
