I am voluntarily participating in physical exercise, which may be strenuous and involve the risk of serious injury, during private or group training sessions organized by Global Bridge Soccer (hereinafter “Company”). I acknowledge that I have consulted, or will consult, with my guardian if I am under 18 years of age before participating.

I understand and agree that my participation in these physical exercise sessions or personal training activities is at my own risk. I recognize that exercise carries inherent risks to both the musculoskeletal system (e.g., sprains, strains) and the cardiorespiratory system (e.g., dizziness, fainting, abnormal heartbeat, shortness of breath, abnormal blood pressure, and in rare instances, heart attack or stroke). I acknowledge that not all risks can be anticipated in advance.

RELEASE AND WAIVER:
For myself, my heirs, executors, administrators, assigns, or personal representatives (collectively, “Releasor”), I knowingly and voluntarily enter into this Waiver and Release of Liability, and I hereby waive any and all rights, claims, or causes of action of any kind arising out of my participation in the above-described activities.

I release and discharge Global Bridge Soccer, its affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors, and assigns (collectively, “Releasees”), from any and all physical or psychological injury that I may suffer as a direct result of my participation in these activities.

I also agree to indemnify, defend, and hold harmless the Releasees from any claims, suits, or actions brought by me or on my behalf, including attorney fees and related costs. I acknowledge that Releasees are not responsible for errors, omissions, acts, or failures to act by any party conducting activities on their behalf.

INTERPRETATION:
Both the participant and Global Bridge Soccer agree that this agreement is clear and unambiguous, and that no outside evidence shall be used to alter or explain its terms. This agreement will be interpreted based solely on the language herein and for the purposes for which it is entered.

By signing below, I acknowledge that I have read, understood, and voluntarily agree to the terms of this Waiver and Release of Liability.

I hereby grant Global Bridge Soccer permission to use my likeness in a photograph, video, or other digital media (“Photo”) in any and all of its publications, including web-based publications, without payment or other consideration.

I understand and agree that all Photos will become the property of Global Bridge Soccer and will not be returned.

I hereby irrevocably authorize Global Bridge Soccer to edit, alter, copy, exhibit, publish, or distribute these Photos for any lawful purpose. I also waive any right to inspect or approve the finished product in which my likeness appears. Furthermore, I waive any right to royalties or other compensation arising from or related to the use of the Photo.

I hereby hold harmless, release, and forever discharge Global Bridge Soccer from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate may have by reason of this authorization.

I HAVE READ AND UNDERSTAND THE ABOVE PHOTO / MEDIA RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE, OR, IF I AM UNDER 18 YEARS OF AGE, I HAVE OBTAINED THE REQUIRED CONSENT OF MY PARENTS/GUARDIANS AS EVIDENCED BY THEIR SIGNATURES BELOW.
