*** FOR THE SAFETY OF CANINE PATIENTS, WE HAVE A LIMIT OF 5 CANINE SURGERIES PER CLINIC. ONCE WE ARE SOLD OUT, THERE ARE NOT ADDITIONAL OPTIONS AVAILABLE.

Reservations will be confirmed once payment for the services are received. You will receive an email confirming the appointment along with further instructions for drop off and surgery day care for your pet. Please add circleareahumane@gmail.com to your contacts list.

If payment is not received within 5 business days, your spot will go to the next person in line.

If your pet already has a rabies vaccination, you must bring the rabies certificate with you to waive a rabies vaccination.

NOTE: A $30 non-refundable deposit + $10 non-refundable admin fee per animal is required at time of signup to reserve your space. Deposit will be applied to total surgery cost.

Liability Waiver

Circle Area Humane Society Liability Agreement: I am the owner to authorize veterinary treatment/ sterilization for my pet. I hereby indemnify and hold harmless Circle Area Humane Society, their employees, board members, and volunteers for any injury or illness to my pet while on the premises or any injury or illness that may occur after my visit as a result of the treatment/ surgery received. I understand the risks inherent in treatment/surgery and sterilization and I accept those risks. I further understand that I must pick up my pet on the day of surgery within one hour of pickup time provided by Circle Area Humane Society. I understand that if my pet is not picked up at this given time (no later than 5:00pm) I will be charged $30 fee by Circle Area Humane Society for the care and boarding of my pet overnight and for each additional day that my pet is not picked up. I understand that the volunteers and employees of Circle Area Humane Society are not vets or vet techs and may not give me veterinary advice regarding my pet. In the event that I need veterinary advice, I agree to contact the RASCAL Animal Hospital. I understand and agree that I am responsible for any charges over and above the payment collected at the time of the scheduling of my pet’s appointment. THIS IS A LEGALLY BINDING CONTRACT
