Description of Material: The material may include, but is not limited to, photographs, audio recordings, and video footage (collectively referred to as “Material”).

Purpose: The Material will be used for purposes including, but not limited to, education, promotion, publicity, fundraising, and increasing awareness of C.A.T.S. programs, activities, and mission.

Scope: This permission extends to the use of the Material in any media format, including print, television, radio, digital, social media, newsletters, press releases, displays, brochures, and other promotional or public materials, now known or developed in the future.

Expiration: This authorization remains in effect until revoked in writing.

Voluntary Consent: Providing this authorization is voluntary and is not a condition for receiving equipment or services from C.A.T.S.

Rights and Liability: I understand the Material will become the property of C.A.T.S. I release and discharge C.A.T.S. from any and all claims or demands arising out of or in connection with the use of the Material, including claims for libel, invasion of privacy, or other rights violations. I understand that I will not receive any compensation for the use of the Material.

Revocation: I may withdraw (revoke) this Authorization at any time by sending a written notice of revocation to the C.A.T.S. Privacy Officer at: 900 Granby Street, Suite 138, Norfolk, VA 23510; Email: hr@atdevicesforkids.org. Withdrawal does not affect any prior use before receipt of revocation.

Acknowledgment: I have read and fully understand this Authorization. I confirm that I am the parent/legal guardian of the child named above.
