Release of Liability
I am a willing participant in the 2027 SFX Super Bowl Cruise & Groove and am authorized to sign on behalf of all travelers listed above on this form. I attest that I have read, fully understand, and agree with the following representations and conditions:
I release SFX Travel and its trustees, directors, officers, employees, agents, volunteers, successors, and assigns from any and all liability for, and waive any and all claims for, injury, loss, or damage in any way connected with my participation in the cruise, including injury, loss, or damage caused in whole or in part by the negligence or other misconduct of the Organization or any of the above-listed organizations or individuals. I am aware of the risks of participating in the event, and assume all risks, foreseeable and unforeseeable, in any way connected with my participation in the event. I agree to indemnify and to hold harmless SFX Travel or any individuals under the umbrella of SFX Travel with regard to any claim or expenses (including attorneys’ fees and other costs of defending any claim by a third party or that I might make, or that might be made on my behalf) in any way connected with a claim. I have read and fully understand, agree to, and accept voluntarily all provisions of this Release of Liability and Waiver, Assumption of Risk, and Indemnification. Although the safety of all travel activities is the primary concern, unforeseen conditions may cause injuries and/or death. I expressly assume the risk of injury, death, and/or illness arising from any cause, and agree to waive the right to pursue any claim against SFX Travel and the persons in charge.
