Enrollment Agreement

Students are responsible for arranging qualified model patients in accordance with course requirements. Required model information and documentation must be submitted by the deadline established by the School prior to the course date. Detailed model eligibility criteria, scheduling expectations, pricing, and instructions are communicated after enrollment and before the course begins.Failure to provide required model information or documentation by the requested deadline may result in administrative cancellation, postponement, or transfer of the student’s enrollment to a future course date at the discretion of the School.Hands-on injectable training is a required component of the approved curriculum and requires student participation in meeting all model requirements. Models are responsible for the cost of any product used during training. Product pricing is determined by the School and is payable at the time of service.

Course scheduling, participation requirements, cancellations, refunds, financial obligations, and completion requirements are governed by the Enrollment Agreement and current Course Catalog. Please review these documents carefully before enrollment.

The tuition displayed below reflects the current enrollment rate for the selected course(s), including any promotional or discounted pricing currently offered by the School. Standard tuition and fees, institutional policies, program requirements, and student disclosures are outlined in the Academy’s approved Course Catalog and Enrollment Agreement. By submitting this form, the student acknowledges enrollment at the tuition rate displayed at checkout.

Hands-on courses require student-provided models and completion of all required documentation prior to the course date. Please review the Enrollment Agreement and Course Catalog before submitting registration.
