Regional Special Vehicles Unit (RSVU) New Member Application
There are several parts to the application that must be filled out in order to process your membership. Please fill out the form in its entirety.

You will need to submit the following information:

1. RSVU Membership Application (this online form)

2. King County Emergency Worker Application (here)

Download this document to your computer, then scan and upload the completed form as a PDF file below. Be sure to initial each page and sign the document. Please note that you do NOT need to include your SSN on this application.

This information will be used by King County Sheriff's Office (KCSO) to complete their database and perform background and driver's license checks, and by the King County Office of Emergency Management to register you as a volunteer emergency worker within King County.

3. Proof of completion for ICS/NIMS 100.C and 700.B.

4. ASI/ROHVA E-Course Completion

We require you to complete the ASI and ROHVA E-Course before you submit your application.

5. Driver License/Insurance/Photo

Additionally, you will need to scan and upload a PDF of your proof of vehicle insurance, and a passport-style photo of you is also required.

Additional information

King County will process your application for a background and driver’s license check. If you do not pass, you will be notified by phone of your ineligibility by the Sheriff’s office. You may re-apply when your records are clear.

ACKNOWLEDGEMENTS AND CERTIFICATION
You understand that within your first year you will need to complete ASI (ATV) and ROHVA (UTV) rider training of approx. 20 hours.

You will be required to attend a minimum of 2 training weekends each additional year of membership.

You understand that you must respond (participate) to 2 missions per year and attend a minimum of 2 meetings per year (flexible during first year). Failure to meet minimum participation requirements may result in review by the Board.

The Washington State Search and Rescue WAC requires an attestation regarding your physical and mental capability to perform an assigned mission or task. Reference WAC 118-04-200. You must check both boxes to complete the attestation.
NOTE: New members will be placed on a 365-day probationary/novice period and during this time the new member must demonstrate his/her professional intentions and capabilities to the organization. The new member must attend meetings, trainings, and missions during the probationary period. At the end of the probationary period, the Board will evaluate the new member. If the new member has shown satisfactory attendance and performance, he/she will be made a RSVU Field team member.
I understand upon acceptance to the Unit, that I must adherer to all policies, training standards, and guidelines of the Unit. Failure to follow those standards may result in disciplinary action, up to and including termination from the Unit, and referral to appropriate authorities if required by law.

I understand that participation in RSVU activities involves inherent risks, including operation of vehicles in hazardous environments, and I voluntarily assume those risks.
