I understand that as a Volunteer, I play an essential role in the success at Prairie Flower and that my attitudes, behavior and responsibilities as a role model are critical to the success and well-being of the girls.
I, therefore, agree to do the following:
• Treat every girl and adult with respect.
• Be sensitive to the needs of each girl participant.
• Respect the places and people with whom I come in contact in my role as an Adult Volunteer.
• Read, understand and follow the Prairie Flower safety policies and procedures posted on the Prairie Flower website. https://prairieflowercamp.wixsite.com/prairieflowercamp/health-safety
• Understand that misuse of drugs and alcohol will not be tolerated, and the use of any during Girl Scout activities will result in the expulsion from the event.
• Smoking is not allowed on the property. If I need to smoke, I will only smoke off the camp property (beyond the entrance to camp) and will not smoke around girls.
• I understand that if I am dismissed due to a serious misconduct, it will be immediate. I will have the option of filing a grievence complaint with the Council.
I have read, understand and agree to follow the rules and procedures of Prairie Flower.

I am attending Prairie Flower Encampment at Camp Lakamaga on May 15, 16, 17 2026. In my opinion, I am physically and emotionally able to participate in Prairie Flower Encampment, subject to restrictions noted in the comment field above in this form. In the event of an emergency and I cannot be consulted, I hereby give permission to the physician via the event director to secure proper treatment for my myself.
