    Thank you for joining us for Spring Camp 2026 at the Central Coast Aquarium! Please complete this form for each child attending. Once completed, you will be redirected to our payment page to complete your registration.

    The following medical information may be necessary in the event of serious illness or accident. Please complete the following sections accurately and truthfully. This form and the information therein will be kept confidential and will only be used to help the staff respond to an injury or illness.

    By signing below, I agree that my child has no health-related issue(s) that should preclude or restrict their participation in this program. I acknowledge that participants are responsible for all expenses in the event that they become ill, injured, or require emergency evacuation or care. All information I have provided is correct to the best of my knowledge.

  • Acknowledgement of Risk:

    I understand that my child will be participating in the Central Coast Aquarium’s camp program, parts of which will be conducted in a wild and natural environment. I further understand that he/she/they will be participating in a program that has inherent dangers including, but not limited to, irregular and unstable walking surfaces (including stairs and sand); wet and slippery surfaces; variable sea and weather conditions; wildlife (terrestrial and marine); low and irregular lighting; equipment; and obstructions. I agree to indemnify and hold harmless from liability the Central Coast Aquarium and Port San Luis Harbor District including agents and employees of those entities by reason of any accident, injury, or damages to persons or property that I, my dependents, or any other person under my custody may suffer.

  • Photo Usage Consent:

    The Central Coast Aquarium may take photos/videos during educational programs that may include your child. This media is used for promotional purposes* only. We do not share names or any personal information. 

    *promotional purposes include social media posts on our Central Coast Aquarium accounts, program brochures, flyers, and occasional website images at www.centralcoastaquarium.org.

    1. You will receive a payment link after submitting this form. 

    2. Follow the link and select each session for which you would like to register.

    *We are kindly asking for electronic payments, if possible. If you would prefer to pay with cash or check, please indicate so below and we will make arrangements to receive your payment.

     

    IMPORTANT: CANCELLATION POLICY

    A one-week minimum notice is required to receive a full refund.

    A five-day notice will receive a 50% refund towards your total payment.

    A three-day notice will receive a 25% towards your total payment. 

    Cancellations with less than three days’ notice are non-refundable.

  • If you prefer to use a payment method other than electronic payment, please indicate so below and we will make arrangements to receive your payment.
  • Scholarship Information:

    Through the support of local grants and private donors, scholarships are available for families seeking financial assistance to send their child(ren) to camp. At this time, we are awarding tiered scholarship options to subsidize camp fees for as many families as possible. If you would like to apply for a scholarship please indicate so below. Award amounts will be determined on a case by case basis.

    A staff member will be in touch with you shortly regarding your scholarship application. If your child is selected as a scholarship recipient, you will receive a custom invoice to complete your registration payment. You do not need to complete your child's payment at this time.
