Minor & Youth Counseling Consent
Counseling People For Christ (C4C Outreach) provides faith-based Christian counseling and pastoral coaching services to minors only with the consent and involvement of a parent or legal guardian.
By enrolling a minor in counseling services, the parent or legal guardian acknowledges and agrees to the following:

I understand that confidentiality for minors is limited. Information may be disclosed to the parent or legal guardian when:
There is a concern for the minor’s safety
The minor poses a risk of harm to themselves or others

Counseling People For Christ (C4C Outreach) provides faith-based support services to families who have experienced the loss of a loved one. Our services are intended to offer practical assistance, spiritual encouragement, and compassionate support during times of grief.
Services may include, but are not limited to:
Assistance with obituary preparation
Support coordinating repast gatherings
Assistance with locating or coordinating musicians and music
General support related to funeral or memorial services
Faith-based encouragement and prayer.
These services are offered as supportive assistance only and do not replace professional funeral directors, legal advisors, licensed counselors, or mental health professionals.
Nature & Limitations of Services
Services provided by Counseling People For Christ (C4C Outreach) are non-clinical, non-legal, and non-medical. We do not provide:
Licensed grief therapy or mental health treatment
Legal advice or document preparation beyond general guidance
Funeral directing or embalming services.
Families remain responsible for all final decisions, contracts, payments, and arrangements made with third parties, including funeral homes, venues, caterers, musicians, or other service providers.

Donations are generally non-refundable, except where required by law.

Donations do not create a client-service provider relationship or guarantee specific outcomes, timelines, or results.
