Please complete this form to authorize credit card payments.
By submitting payment via Square Invoice, I hereby authorize Studio 944 to charge my credit or debit card for the full amount specified on the invoice. I represent and warrant that I am the authorized cardholder and that I have the legal authority to approve this transaction.

I acknowledge and agree that all payments made are non-refundable and non-transferable, and that the entire remaining balance is due no later than fourteen (14) days prior to the scheduled event date. I further agree that all payments must be submitted using the same card initially authorized, unless a separate and additional credit card authorization form is completed and approved by Studio 944.

This authorization shall be retained by Studio 944 for purposes of transaction verification and dispute resolution, including the defense of any chargebacks deemed fraudulent or in violation of these terms. No charges beyond those expressly authorized on the invoice will be processed without additional consent.
