    Whether this is your camper’s first year or a return to Ohdaka, this form helps us gather the information we need to support your child and keep camp running smoothly.

    • Camp is nearing capacity at many grade levels as there are now limited spots available. If you are registering siblings OR hoping to register your camper with a friend, please check our availability below to ensure there are spots available for your camper(s).

      To add your camper's name to a waitlisted group, please complete this registration form and pay a $25 deposit*. We will notify you via email if/when a space opens up for your camper.

      *Deposit is fully refundable if we cannot accomodate your camper this summer. You would then have the option to role your deposit to 2026  or receive a full refund.

      Our mission is to connect communities through outdoor recreational and educational opportunities. We create hands-on experiences for youth and families that build teamwork, communication, connection to the natural world, and a commitment to building better communities.

    • Camp Details: 

      Campers must meet the age requirement as of the first day of camp.

      Camp Details: June 22–26, 2026 | 9:00 AM–3:30 PM
      Ages: Pre K (age 4+), Kindergarten through 8th (use the grade they will be in in the fall of 2026)
      Location: Oxbow Regional Park, Gresham
      Cost: $350 - includes required $65 bus fee

      Transportation for 2026:

      Ohdaka will use camp bus transportation for daily travel to and from camp
      Pick-up and drop-off: MHCC athletic (Yoshida Event Center) parking lot
      Morning drop-off: 8:00–8:15 AM
      Afternoon pick-up: 3:45–4:00 PM
      The weekly bus fee is included in the camp registration fee

      In limited, pre-approved circumstances, parent/guardian transportation directly to Oxbow Park may be arranged.

      Additional fees may apply for replacement items (such as a forgotten lunch or camp t-shirt), camp store purchases, or late pick-up. See the Parent Packet for details.

    • Camper Abilities, Behavior & Community Expectations

    • Every camper is unique. The information below outlines general expectations and helps ensure a safe, supportive, and positive experience for your child and the entire camp community.

    • Camper Abilities 
    • At Ohdaka Day Camp, we strive to create a welcoming and inclusive environment for all campers. If your camper has any health, mobility, learning, or disability-related needs, please share that information with us in advance. We will make every reasonable effort to support campers within the scope of our day camp program.

      In addition to meeting the age requirement, campers must be able to meet the following expectations with or without reasonable accommodation, in an outdoor, group-based setting:

      Campers must be able to:

      • Participate in outdoor activities for most of the day, including walking up to 1–2 miles on grass, dirt trails, gravel, and paved paths
      • Carry a small backpack with personal items (lunch, water bottle, extra layers)
        Communicate basic needs to staff (bathroom, water, feeling unwell, needing help)
      • Manage personal care independently, including restroom use, changing clothes if needed, and managing personal belongings
      • Follow simple and multi-step directions in group settings
      • Participate safely in a group environment with age-appropriate supervision ratios
      • Use age-appropriate strategies to regulate emotions and behavior in a busy, community-based setting
      • Engage respectfully with peers and staff and refrain from harming themselves or others


      Ohdaka Day Camp is not equipped to provide one-on-one staffing, reduced supervision ratios, or personal caregivers. We may not be able to support campers whose needs require constant individualized supervision or whose behaviors significantly impact the safety of themselves or others in a group setting.

      If you are unsure whether Ohdaka Day Camp is the right fit for your camper, or if your camper has needs that require additional discussion, please contact us at office@shootingstaradventures.org before completing registration. This allows us to partner with you thoughtfully and determine the best course of action.

       

    • Camper Behavior & Community Expectations 
    • At Ohdaka Day Camp, clear and shared expectations help create a safe, supportive, and fun experience for all campers.

      Families are expected to review the Ohdaka Camper Code of Conduct and the kid-friendly Ohdaka Camper Promise with their camper prior to the start of camp. Both will be included in the Parent Packet and available at the link below.

      On the first day of camp, campers will review expectations together and take the Ohdaka Camper Promise as a group, helping set a positive tone for the week.

      These shared expectations support kindness, safety, participation, and respect within our camp community.

    • Camper Code of Conduct

      (Required for all campers in 2026)


      Please complete the Camper Code of Conduct by clicking the link above. The form will open in a new window—once completed, you may return to this registration screen.

      If your camper is not with you at the time of registration, you may complete the form on their behalf. Please be sure to review the Code of Conduct with your camper before camp. A copy of your completed form can be downloaded for your records.

      If needed, the Camper Code of Conduct can also be completed after registration. It will be linked on the submission confirmation screen and included in your confirmation email.

  • Demographics

  • The following questions help Shooting Star Adventures understand who we serve and are often required for grant funding and program evaluation. This information supports scholarships, transportation, and program access.

    These questions are optional and confidential. Responses are reported only in aggregate, never used to determine eligibility or participation, and choosing not to answer will not impact your camper’s experience. You may select “Prefer not to answer” for any question.

  • Thank you for helping us collect this information. Demographic data allows us to demonstrate community impact, apply for grant funding, and expand access to outdoor programs for youth and families. All responses remain confidential.

  • Camper Information

  • We are sorry but only returning campers with Ohdaka or Camp Two Roads who have applied and been accepted may participate in our teen Galaxy Guide program. Please contact office@shootingstaradventures.org for more information.

    We are sorry, but your unfortunately your camper's age level is currently full. To be put on our cancellation waiting list, please continue to complete this registration form.

    A refundable $10 deposit is required to join the waitlist. Campers are offered spots in the order they register, based on unit availability.

    Families will be notified as soon as a spot becomes available. If your camper is offered a spot, the registration fee will be charged at the rate in effect on the date of the original registration.

    If a spot does not become available, the deposit will be fully refunded.

    • Make sure there is nobody else in the camera frame.
    • Crop image to head and shoulders.
    • Remove glasses and hats.
      Camper is looking straight at the camera, no funny faces.
    • Make sure there is enough light.
    • School photos are a good choice.
  • Health Needs

    Providing complete and accurate health information helps us care for your camper and respond appropriately to their needs during camp.
  • Please notify us of any health, mobility, or disability-related needs your camper may have while attending camp. This information allows us to plan appropriately and support your camper’s safety and well-being. We will make every reasonable effort to accommodate camper needs within the scope of our day camp program.

    Shooting Star Adventures does not discriminate against campers with disabilities; however, we may not be able to support campers whose needs require one-on-one supervision, personal caregivers, or significant individualized support beyond what can be safely provided in a group day camp setting. In some cases, we may need to speak with you directly about your camper’s specific needs before registration is finalized to ensure Ohdaka Day Camp is the right fit for your child.

  • Because many of our counselors are high school students, it’s important that we ensure our staff are able to safely and appropriately support your camper’s needs within our current camp model.

    At this time, campers whose needs include significant physical limitations, PTSD, emotional dysregulation (ED), oppositional defiant disorder (ODD), or other intensive behavioral concerns may require supports beyond what we are able to provide in a group day camp setting.

    If your camper has needs that may fall into one of these areas, please contact the Ohdaka Camp Director at 971-328-1653 before proceeding with registration so we can discuss your camper’s needs and determine the best next steps together.

  • Over-the-Counter Medication Release

    The Oregon Health Authority require camps to obtain written permission to administer specific over-the-counter treatments used for common ailments such as headache, poison oak, upset stomach, or diarrhea. Dosage will be administered according to official directions written on the product unless otherwise directed by official physician instructions for the following OTC meds our Health Hut has on hand.

    You can also complete/update this form after you complete registration up until 1 week prior to camp - it will be linked on the submission screen, in your confirmation email and found on our website.

  • Parent/Guardian Information

    This person will be listed as the participant's main contact in case of emergency. These names CANNOT also be the emergency contact person.
  • Additional information is needed. Please fill out the Case Worker name and contact information. We must verify all emergency contact information is accurate before we process this registration.

  • Emergency Contact Information

  • Please list the first and last name of every person authorized to pick up your camper - we check IDs against this list everyday to ensure the safety of our campers.

  • Please provide information for at least one additional emergency contact or authorized pick-up person.

    In the event of an emergency, we will first attempt to contact the parents/guardians in the order listed on this form. If we are unable to reach them, we will then contact the additional emergency contact(s) you provide.

  • Please choose a different emergency contact. It cannot be a parent/guardian listed previously.

    I understand that a non-refundable deposit must be received for each registration in order to secure a participant's spot at camp. Payments processed through STRIPE. The charge will show up as ShootingStarAdventures. Registrations received without a deposit will be put on the waitlist until payment is received.

    • Payment Options 
    • All outstanding balances must be paid by June 1st. Choose from the following options:

      Payment in Full: 

      Your camper’s spot is reserved immediately when the full registration fee is paid.

      No additional payments will be required unless you choose to add optional items later (such as camp bus transportation, camp store credit, or merchandise).

      Payment Plan: 

      Choose one of the equal monthly payments options below.

      • Your camper’s spot is reserved once the first payment is received
        • 2 months at $175
        • 3 months at $117
        • 4 months $87.50
        • First payment due at registration, includes the non-refundable deposit
        • Second payment due the following month

      Deposit Only – Financial Aid Applicants:

      A $50 deposit option is available only for families applying for financial aid.

      This deposit holds your camper’s spot while your financial aid application is reviewed. Remaining balances will be due according to the payment schedule outlined in your financial aid agreement.

      Waitlisted Campers

      A $10 refundable deposit is required to join the waitlist.

      Families will be notified as soon as a spot becomes available. If your camper is offered a spot, the registration fee will be charged at the rate in effect on the date of the original registration.

      If a spot does not become available, the deposit will be fully refunded.

    • NSF Policy 
    • NSF - Non Sufficient Funds

      If a payment fails, the system will notify you and automatically retry.
      After 3 failed attempts, a $25 NSF fee will be added.
      If payments fail for 2 consecutive months, you will be invoiced for the full remaining balance.

      Delinquent Accounts – If payments are not up to date, your camper’s registration may be placed on hold and moved to the waitlist, which could result in losing their spot at camp. In that case, refunds will be issued in accordance with the camp’s cancellation policy, with the $50 deposit remaining non-refundable.

    • Financial Aid 
    • Financial Aid is no longer available for the 2026 Summer Session. Please check out the personal fundraising options listed on our Financial Aid  page  for alternative ways to cover your camp registration.  (Open this link in another tab to avoid restarting your registration.)

    • Financial Aid Requests due withing 30 days of registration - To be eligible you must: 

      • Select "Yes" for financial aid requested below.
      • Select Payment Plan - Financial Aid Application Pending for the remaining balance.
      • Complete this registration prior to May 15th and pay the deposit.
      • After completing registration - fill out the Campership Application. You must fill out this application within 30 days to prevent your account from defaulting to the lowest priced monthly payment plan still available at time of registration.


      If you are awarded Financial Aid and you decline it prior to the deadline you will receive a full refund of registration monies already contributed, including your deposit.

      In the event you cancel your reservation, refunds are defined below:

      • Notice of cancellation must be submitted in writing to finance@shootingstaradventures.org.
      • Refund will be issued in the same manner payment was made. If the payment was made by credit card, then the refund will be credited back to the card.
      • 100% Refundable registration will be reimbursed (minus the $50 nonrefundable deposit) if written notice is received prior to May 1st.
      • 50% of refundable registration will be refunded between May 1st and May 31st. 
      • Cancellations received on or after June 1st will forfeit full registration costs.
      • Please allow 4-6 weeks to receive your refund.
      • Participants who are asked to leave or choose to leave during a camp session will not receive a refund.

    • Digital Image and Comprehensive Liability Release 
    • The named camp participant and his / her parent or guardian (if applicable), hereby grant permission to Shooting Star Adventures, Inc. to be photographed and / or otherwise have images or voice recordings made (including but not limited to digital photographs, video or digital moving images, or voice recordings) for publication or promotional purposes. I authorize the above named to be used in any medium (including but not limited to print media, newspaper, television, video, motion picture, social media or internet website). I consent to the use of my or the program participant’s name or interview comments in connection with Shooting Star Adventures, Inc. publication or promotional purposes in print media, newspaper, television, video, motion picture, or Internet publication. In addition, I / we waive the right to inspect or approve draft or finished products, including written or electronic copies, wherein my likeness appears. I / we waive any right to royalties or other compensation arising or related to the above-described actions. I / we hereby hold harmless and forever discharge Shooting Star Adventures, Inc., and its affiliates and volunteers from all claims, demands, and causes of action which I, our heirs, representatives, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization. To the fullest extent permitted by law, I hereby waive any rights I may have to sue Shooting Star Adventures, Inc., its affiliates, employees and/or volunteers involved with the coordination and execution of programming.  I understand this permanent waiver of my rights to sue Shooting Star Adventures, Inc. for any reason stands with respect to personal injury, communicable illness, property damage, emotional affects, civil rights violations, or any other experience of concern as a result of my child’s participation in programming. I hereby release Shooting Star Adventures, Inc. from any and all liability in every case and agree to use and exhaust any and every insurance policy unrelated to Shooting Star Adventures, Inc. prior to filing a claim associated with Shooting Star Adventures, Inc. or the liability coverage of their programs.
