Appointment Entry & Title Confirmation Acknowledgment

By submitting this form, I understand and acknowledge that:

• A $150 non-refundable Appointment Entry Fee is required at the time of application.
• If selected for a state or international title, my $650 Registration Fee will be waived. I will receive title confirmation and an invoice for the $1,200 International Fee within 48 hours. It will be sent to the email address submitted on this form. International Fees can be paid in three installments. 
• My official crown and sash will be ordered once the first International Fee installment ($400) is received, at which time my state title is considered fully confirmed.
• State titles are awarded on a rolling, first-approved basis.
• A maximum of two titles per state, per division will be awarded:
