Use this form to log your volunteer service hours for the 2026 Service Year (January 1–December 31, 2026). Logging hours helps MWoV make service visible and prepares volunteers and organizations for community recognition.

Community Recognition & Awards will be presented in March 2027.

How to use this form (keep it simple):

Submit one entry per organization you served.
You may submit after each service date or once per month—whatever helps you stay accurate.
Keep descriptions short and clear (1–2 sentences is enough).
Hours may be verified by the organization(s) served prior to certification and recognition.
Community awards and recognition will proceed regardless of the current federal PVSA award-ordering pause.

MWoV does not sell, rent, or share personal information, including personally identifiable information (PII), with outside parties except as required for legitimate program administration or legal compliance.
