INSTRUCTIONS

Volunteers carry more than tasks—they carry trust. Certifying service hours is one way to honor the time people give and the responsibility your organization holds.

Use this form to certify volunteer service hours for the 2026 Service Year (January 1–December 31, 2026). Organizations may certify hours monthly or quarterly—whatever keeps your records accurate and your volunteers recognized.

Community Recognition & Awards will be presented in March 2027.

If your organization is tracking service now, you are already building toward that moment.

Two ways to submit hours:

Enter hours directly in the form (best for a small number of volunteers), or
Upload the MWoV Certification Spreadsheet (best for multiple volunteers or internal spreadsheets).

Please submit one certification per reporting period (monthly or quarterly).
If you certify for multiple programs or locations, submit separately as needed for clean records.

Organizations certifying 10 or more volunteers may use the MWOV Certification Spreadsheet to report service hours in bulk.

Download the MWoV Certification Spreadsheet, complete one row per volunteer, then upload the completed file below.

Please be sure that if you use the spreadsheet that you include ALL of your volunteers for the service period on that one spreadsheet. If you have more than 100 Volunteers to certify, please use additional spreadsheets to record their service/contribution.
