In Home Training Session Registration Contract Form and Release

THIS FORM IS A LEGAL CONTRACT WITH LAURIE TUTTLE DOG TRAINING LLC - PLEASE READ AND FILL OUT BEFORE SUBMITING FORM.

Your Training Session Date and Time will be scheduled 24- 72 hours after submitted. Laurie will call, if for any reason she does not contact you, please call and leave a message if needed at 219-776-7312

Dog Training Agreement, Release and Indemnification Agreement

I, the undersigned (also refered to as "Owner") hereby knowledgeably enter this Dog Training Agreement, Release and Indemnification Agreement ("Agreement") with Laurie Tuttle Dog Training, LLC (LTDT) on behalf of myself and any I effectively designate as my agent(s) by relying upon such individual(s) to assist me in arranging for the delivery of services described herein ("Owner's Agents"). Owner Consents to the delivery of services from LTDT to Owner and Owner's Pet(s), which may consist of training, pet care, and/or boarding. Owner acknowledges and understands that there are certain risks involved in pet training, boarding, and care and, by signing this Agreement, accepts all such risks, according to the terms of this Agreement. Owner hereby waives, releases, and discharges any and all claims for from any and all manner of damages, claims, loss, liabilities, costs or expenses, causes of actions or suits, whatsoever in law or equity, (including, without limitation, attourney's fees, which Owner and/or Owner's Agents may have, or which Owner and/or Owner's Agents may subsequently have against LTDT, including but not limited to such claims against employees, independent contractors, representatives, agents, officers and managers acting on behalf of LTDT, including but not limited to Laurie Tuttle (collectively referred to as "LTDT Agents"). Owner releases, indemnifies, and agrees to hold LTDT and LTDT Agents harmless from any and all manner of and from any and all manner of damages, claims, loss, liabilities, costs or expenses, causes of actions or suits, whatsoever in law or equity, (including, without limitation, attourney's fees, any and all claims for personal injury and/or property damage to Owner, Owner's Agents and any member of owner's family and/or other persons-accompanying Owner to any training session or function on the property or surrounding area thereto, including costs of such defense.

Owner understands and agrees that Owner is solely responsible for Owner's pet and its behavior. If Owner's pet hurts another person, pet and/or property, Owner shall be solely responsible to pay all costs caused by and related to such behavior. Owner understands that use of all LTDT equipment comes with risk to Owner and Owner's pet. Additionally, Owner acknowledges that there are dogs of all sizes and temperaments, including aggressive pets in the training classes and/or on or about the LTDT business premises. Furthermore, Owner and/or Owner's pet could come into contact with contagious diseases (animal or human), and though rare, could be bitten, attached, or seriously injured. Owner's dog may escape from the premises and be lost or injured. Owner agrees and stipulates that LTDT, LTDT Agents, and the owner of LTDT's business premises are not and shall not be held liable for any costs or expenses incurred as a result of Owner and/or Owner's pet's participation and Owner will look solely to the owner of the pet causing injury for damage compensation. LTDT and LTDT Agents reserve the right to refuse or terminate services of any pet at any time, for any reason. 

Owner also understands there is no guarantee of achieving training goals despite the efforts of LTDT. At no time does LTDT guarantee that: dog will never bite; dog will not be dangerous in the future; dog will not exhibit other behavioral problems: or, results of the training will last for any particular amount of time. 

Owner understands that payment is due in full upon completion of this form and understand that NO REFUNDS will be made, unless specific arrangements are made by LTDT in writing. Owner understands that classes/training sessions are filled at a first come, first serve basis. Owner understands that Owner will be committed to the class/training session upon signing up and understands that, due to time constraints, there will be NO MAKE UP SESSIONS. Owner understands that all missed sessions will be caught up in the next class after the missed class. Owner understands that if a female comes into season Owner will receive a 3-week credit and that credit must be taken at the very next training session. This credit applies to the same pet and Owner only. Owner understands that if the Owner doesn't give 24 hours' notice by the options of phone/text/voicmail for cancellation of Home Training Sessions:then owner will loose the full deposit for that session in order to make up for travel/training loss. Owner also agrees there will be a returned check fee in the amount of $35. In the event of default of the terms and conditions of this Agreement, Owner agrees to pay any outstanding balance owed to LTDT together with interest at the rate of ten (10.00%) percent per annum, reasonable attourney fees, court costs and other costs of collection that may be incurred by LTDT. The parties agree that the courts of Lake County, Indiana for Indiana clients - and Houston County, Alabama for Alabama clients. (including City courts, Superior Courts, Circuit Court, or Small Claims Courts) shall have exclusive jurisdiction for any dispute arising pursuant to the terms and conditions of this Agreement.

CANCELATION POLICY:

To cancel and/or reschedule an appointment:
You are required to do so by PHONE/TEXT/VOICEMAIL(no emails).
You must contact Laurie Tuttle Dog Training LLC within 24 hours of your appointment's DAY and TIME, in order to keep your full price of the training session for that appointment.

Failure to do so will result in you loosing the FULL price of the training session for that visit, no exceptions!

More than 3 - last minute cancellations and/or rescheduling of your appointment outside of the 24 hour cancellation policy will result in termination of this contract/services with NO REFUNDS, INCLUDING PACKAGES, NO EXCEPTIONS!
- 24 hour means- 24 hours of the DAY and TIME of your appointment. NO EXCEPTIONS!

Example: Appointment is on Tuesday June 30 at 6 pm, you must Cancel BEFORE Monday June 29 at 6 pm.

Excessive cancellations and/or reschedulting IN THE TIME ALLOWED of the 24 hour cancellation policy will also be considered for POSSIBLE termination of this contract/services with NO REFUNDS, INCLUDING PACKAGES, NO EXCEPTIONS!
- 24 hour means- 24 hours of the DAY and TIME of your appointment. NO EXCEPTIONS!

Please be considerate!!!!
If you know you need to cancel ahead of time, please let Laurie know as soon as possible so she may open up your spot for another client who may be waiting for training.

Refund Policy:

PLEASE be commited to training BEFORE signing up!!

Once you fill out the contract form and prepay for your session, there are no refunds, for any reason, no exceptions!

When purchasing training sessions, training consults, and/or packages-

ALL sessions must be used up before the end of 6 months of the date of purchase.
Any unused sessions will be forfeited.

PAYMENT POLICY/OPTIONS:
All clients must pre pay for each visit(s).
Payment is due at time of scheduling the next visit - unless agreed upon by Dog Owner and Trainer.
If payment is not received within 24 hours of scheduling your appointment, your session will be cancelled. To resume training you will need use the form online/pay to get a new appointment.
Online payment by Credit Card is accepted for all new and established clients.

For established clients - cash and check will be accepted but pre-paid is still required at the time of scheduling an appointment.

Advertising & Media Release:

Owner agrees that LTDT has Owner's written consent to publish, print, post, "tag", and share any photo or video containing Owner, Owner's Agents, and/or pet on any social media, website, publication, and/or for advertising purposes related to LTDT.
No Personal information will be given out other than Owner's first name, pet's name, and class that was attended unless additional verbal authorization is given.
***Behavior case clients will be asked to give LTDT additional verbal consent by owner to share a generic case history before using for advertising, social media, etc.

1. By submitting this form, you acknowledge that you have read and understand the Agreement above.
2. Payment is due upon completion of this form.

What to have for your training session:

1. Copy of Vaccination records

2.Notebook/pen for the each session.
Please have your questions written down ahead of time, I will happily answer  questions. You will want to also take notes as we can cover a lot of different material in one session.

3. We recommend having several kinds of soft training treats & 1-2 high end treats cut up in very TINY peices(approx. 50-100) for each session.
Examples of high end treats: Chicken, string cheese, kosher all beef hot dogs, freeze dried treats, soft jerkies made for dogs, etc.

DO NOT USE KIBBLE!

A training pouch is not required but it is highly recommended for ease during training.

We also suggest on training days to cut their meal, before or after training, in half. If their feeding time is close to the training time, please wait until after training to feed them their meal.

4. Collar and Leashes:
A regular 6ft leash is required.
A flat collar or harness that is well fitted to prevent escape.
A regular collar is required even if you use a harness. Please bring both.
Martingale collars(anti-slip) are accepted as long as they are being used for safety and not correction purposes.

Training Tip:
On days you have training: Please do not schedule vet visits, grooming, or day care visits with your dog. Your dog will be too tired and may not even feel well enough for training. It can create additonal stress/emotional distress for your dog which can have behavioral consequences.

 

The following are strictly prohibited and will NOT be allowed in our training:
Flexi/retractable leads, bungee, or chain leads. 
Choke chains, pinch/prong collars, and electronic/shock collars.
Head halters are strongly discouraged.

While these items seem like harmless and quick solutions to training, they actually can cause injury to yourself and/or your dog.
Some of these tools can lead to cause long term emotional/behavioral harm in dogs.
