CHURCH OFFICIAL VOCATIONS APPLICATION PACKET

Thank you for your interest in becoming clergy within the United States Old Catholic Church and/or a Benedictine Brother or Sister with the Old Catholic Benedictines of Immaculate Mary within in the USOCC!

We welcome your application and look forward to getting to know you. The following is a list of things that must be completed along with the application within this packet. Your application is not considered complete and cannot be acted upon or reviewed until all materials listed here have been provided to us.

Please fill out the application in this packet and send it to: The USOCC Vocations Director will acknowledge receipt of the application and also answer any questions you may have regarding it. Use a separate sheet of paper to answer the questions in the application. It is not necessary to repeat the questions in your answer sheet, but PLEASE number your responses to the align to the corresponding question so that we can match your answers accordingly.

As you will see, the application contains a section at the end for signatures. Please initial and sign where indicated. Make a copy for your own records and mail the original to:

4. Certificates and Paperwork Required

Please be prepared to send us the following:

A copy of your sacramental papers, including Baptismal Certificate (for any Christian religion), your Confirmation Certificate (Catholic only), any ordination papers (for any religion), & excardination papers (for Catholics only A copy of any educational degrees or certifications you've received from college, graduate school, or from vocational or technical schools. Any and all College transcripts, graduate school transcripts, or continuing education classes (if they are in theology or a similar field Any certificates for ministerial competencies or accomplishments.

Once the application is received, you will receive a link to follow in order to take an online personality test. We will share the results with you once the test is complete. This is not a formal psychological evaluation. It is just a tool to help us understand your way of engaging the world and may be helpful in determining what ministries you are best suited for.

One must be 21 or older in order to be ordained to the priesthood. However, applicants to ordination may be 18 with the intention of attending seminary so they are prepared for ordination at 21.

Applicants to the Benedictine Order who are not seeking ordination, must be 18.

All applicants must have a High School Diploma, GED, or equivalent.

Once accepted into the program, applicants are required to take an entrance exam for seminary. This is only to determine placement. Once an applicant is accepted into the Vocations Process, they are also accepted into St. Mary's Old Catholic Seminary.

The USOCC does not discriminate based on age, race, sexual orientation, gender identification, marital status, citizenship, physical abilities, etc. All applicants are considered equally based on their fitness for the priesthood and/or the communities they seek entry to.

List any educational degrees (accredited, unaccredited, or honorary) that you have received and/or any continuing education credits, classes, or training you have completed. You will be asked for unofficial transcripts for enrollment in our seminary.

Have you ever served in the Armed Forces? If so, please tell us which branch, rank at discharge, reason for discharge, and any special training you received during your time in the military.

If there is anything else you'd like to share, like businesses you've started, special life experiences you've had, pilgrimages or trips you've taken, please list them.

To assist you in choosing, the purpose of these references is to learn things about you that you may not think to tell us, to determine how you interact with others, and how others perceive your gifts and strengths. Other people often have a better view of our vocation than we do ourselves.

I understand that only those with direct involvement in the vocations process will view this record and that no portion of it or of the answers to the preceding questions will be released to the public nor to members of the church not involved in the Vocations or seminary decision-making process. I understand that, while the conviction of a crime is not a cause for automatic rejection of my application, a positive finding of a crime that I have not disclosed or lied about in part or in whole is a cause for immediate dismissal of my application.

USOCC. I further consent to the release of the results of same to the Vocations Director, the

Chancellor of the Seminary, and the Presiding Bishop of the USOCC. I understand that this is a medical test and that I will receive a copy of my HPA rights and sign a HPA form. I further allow that a copy of this report will be kept securely within my private file. I acknowledge that my refusal to cooperate in this examination will result in the immediate suspension of this process and the termination of my application.
