How to Save a Life Transportation (NYC)
PART 1: INDEPENDENT CONTRACTOR (1099) DISCLOSURE
PLEASE READ CAREFULLY: By applying, you acknowledge that you are seeking a partnership as an Independent Contractor (Form 1099), not as an employee. In accordance with the NYC Freelance Isn't Free Act, the following terms apply to this service relationship:
1. Contractor Responsibility & Expenses
As an independent business owner, you are responsible for the "tools of the trade." This includes:
Vehicle: Providing a TLC-licensed vehicle that meets NYC medical transportation standards.
Costs: All fuel, tolls, maintenance, repairs, and parking/traffic fines.
Taxes: Responsibility for all federal, state, and local self-employment taxes (No taxes are withheld by the company).
Equipment: A modern smartphone with a data plan for dispatching and communication.
2. Mandatory NYC TLC Credentials (2026 Standards)
You must maintain the following at your own expense to remain eligible for trip offers:
TLC Driver License: Must be active and in good standing.
Class E License: New York State Driver’s License (or equivalent).
Drug Testing: Current annual TLC-approved drug test on file.
WAV Training: Completion of the Wheelchair Accessible Vehicle (WAV) training course.
Medical Fitness: A valid TLC Medical Certification Form completed by a licensed physician.
3. Commercial Insurance Requirements
Contractors must carry insurance that meets the NYC TLC minimum liability limits:
Liability: $100,000 per person / $300,000 per occurrence.
Property Damage: $100,000.
FH-1 Certificate: Proof of current coverage must be provided prior to the first trip.
