By signing, I acknowledge the following: My child will be in the care of Coach Kyle and his staff. We will always have kids within the legal ratio set by ODJFS, or better. They will be taking part in hiking, sporting, theatre, art, and many different types of activities. I have carefully reviewed and answered all of the permissions. First aid may be applied by any of the adults at the camp, and in the case of serious injury, I allow for the staff to call emergency services. If an injury occurs at the creek, I give permission to Coach Kyle to transport my child (with booster seat if necessary) back to Our Lady of Peace School.
