Doggie Day Out Agreement -
I agree to the following as a condition of obtaining temporary care of the field trip dog:
I am at least 18 years old or older and will be the primary handler for this dog.
I understand that the assigned dog in my care legally belongs to Pets Lifeline and that this excursion does not meet the requirements of an adoption, sale, or transfer of ownership to me.
I agree to keep the dog in my care and guardianship during the field trip and not assign responsibility of this dog to any other individual.
I will provide all care necessary to ensure the safety, health and emotional well-being of the dog assigned to me. If at any time I am unable to meet these requirements, I agree to immediately return the dog to the Hawaiian Humane Society.
I agree to abide by all state and county laws relating to dogs and all Pets Lifeline policies.
I agree to ensure that the dog is always kept on a leash, wearing a Pets Lifeline collar, harness, air tag and "Adopt Me" vest. I understand that I cannot take this dog to any off-leash locations (e.g. dog parks) and that this dog may not be off leash under any circumstances.
I agree to never leave the assigned dog alone in a vehicle or otherwise.
I agree to never leave the assigned dog unsupervised with children.
I agree to only feed the assigned dog the provided treats and not human food.
I agree to ensure the dog has access to fresh water and I will monitor for signs of overheating.
I agree to not allow my assigned dog to meet another animal and agree to adhere to best practices when introducing the dog to new people.
I agree to ensure the safety of the dog during transportation by using the seat belt provided for all trips to and from any destination. The dog will not roam freely in the car or be transported in an open convertible, Jeep or the bed of a truck.
I understand that I cannot take this dog to any location that may have animals off leash (e.g. dog parks).
If the dog becomes lost, I will immediately inform Pets Lifeline by calling this emergency number: 707-996-4577. I also agree to canvas the area the dog was lost in and ensure to be fully cooperative with Pets Lifeline to find the lost dog.
If at any time the dog becomes sick or injured while in my care or Pets Lifeline requests to evaluate the condition of the dog, I will bring the dog back to Pets Lifeline immediately. I agree not to administer any medication except under the direction of Pets Lifeline.
I agree to immediately return this animal to Pets Lifeline upon fulfillment of the field trip at the time specified for return, or upon request by the organization. Pets Lifeline may terminate this agreement at any time in its sole discretion. I understand that the dog will be seized from my custody if I do not honor these contractual obligations and that I can be prosecuted for theft if this dog is not returned as required by this agreement.
I understand that this dog has undergone a basic health examination and behavior observation period and Pets Lifeline has fully disclosed all it knows about the dog. I accept the dog as is and assume all risk that may arise, including the risk of damage or injury to person or property caused by the dog. I hereby agree to indemnify, hold harmless, release, and defend Pets Lifeline from and against any and all actions, claims, damages, disabilities and expenses that may be asserted by any person or entity arising out of connection with the medical condition, behavior or care of this dog.
