I, the undersigned, voluntarily consent to receive nail care and/or massage services provided by Peaches Nail Lounge, including but not limited to manicures, pedicures, senior nail care (including thickened or fungal nails), and circulatory foot and leg massage services.
I understand and acknowledge that all services provided by Peaches Nail Lounge are non-medical and are intended for cosmetic, comfort, and relaxation purposes only. I understand that Peaches Nail Lounge does not diagnose, treat, or cure any medical conditions.
I affirm that I have fully disclosed any and all known medical conditions, allergies, skin sensitivities, circulation issues, diabetes, neuropathy, infections, open wounds, or other health concerns that may affect my ability to safely receive services. I understand that it is my responsibility to immediately inform the technician of any discomfort, pain, or concern during the service so that services may be adjusted or stopped if necessary.
I understand that nail care and massage services involve inherent risks, including but not limited to skin irritation, soreness, cuts, or aggravation of existing conditions. I knowingly and voluntarily assume all risks associated with these services.
In consideration for receiving services, I hereby release, waive, and discharge Peaches Nail Lounge, its owner, employees, contractors, and affiliates from any and all liability, claims, demands, damages, or causes of action, whether known or unknown, arising out of or related to services rendered. I agree that I will not hold Peaches Nail Lounge responsible for any injury, discomfort, or adverse reaction resulting from services provided, so long as services are performed with reasonable care and within the scope of practice.
I understand that no guarantees or warranties have been made regarding the results or outcomes of services, including improvement of nail conditions or circulation.
I further grant permission to Peaches Nail Lounge to take photographs and/or videos of my hands, feet, lower legs, and/or experience, before, during, or after services for educational and marketing purposes, including use on websites, social media, and promotional materials. I understand that no personal identifying information will be shared and that this consent may be revoked in writing at any time.
I certify that I have read this document in its entirety, fully understand its contents, and sign it freely and voluntarily.
