This is the sign-up form for the junior showcase 2026. Please read this in full and provide all of the information required. If you have more than one child taking part, you will need to complete a form for each child. The junior show will take place on Sunday 14th June 2026 at The Sandpit Theatre in St Albans. There will also be dress/tech rehearsals on Saturday 13th June but these will fit around Saturday classes so they can attend their classes at CMG for an extra in-studio rehearsal before coming to the theatre. Students can only take part in the showcase if they attend the dress/tech rehearsal at the theatre on Saturday 13th June 2026.

FIT TO PERFORM DECLARATION

By completing and submitting this form you confirm that your child is fit and well to participate in the showcase and should anything change between now and the showcase, you will inform us immediately.
PHOTO & VIDEO CONSENT

The show will be photographed and filmed. The video will be made available to you via an online link free of charge. Photos will be avaiable to purchase. You understand and agree that images and videos from the production may be used for promotional material both online and in hard copy format to support the ongoing promotion of the businesses and to celebrate their performances.These will be used for CMG Arts, Georgina Pay School of Dance and Cassana Performance Academy. You agree to have your childs' photo included in the programme for the show along with their first name.  If you are not happy with the photo and video consent as outlined above you must put this in writing to the following email address by no later than 1st February 2025: hello@cmgarts.com Please sign below to confirm you have read and understand all of the information provided on the form and are happy to register your child to take part in our showcase in June 2026.
