Please complete this form and bring your artwork to the gallery on Intake day. Labels will be attached at the time of check in. There are no fees to particpate in this show!

All artwork submitted MUST be properly wired (canvases) or framed and wired (all other 2D medium). No artwork will be accepted if wiring is not securely attached to the frame (not the backing). Questions can be directed to Rupam Varma at general@minthillarts.org.
