By signing this registration form, the registrant agrees to abide by the rules and regulations of Markdale Minor Softball, its governing bodies and any affliated leagues and/or tournaments.

I herby release Markdale Minor Softball, The Municpality of Grey Highlands, all coaches, intructors and officers from all claims of damages resulting from any accident which may arise from the registrant's particpation during any part of such activities or by reason of the provision/administration of medical care to the registrant.

I consent to the publication of the resigstrant's first name and last name and/or photograph in local newspapers, Markdale Minor Softball's website, and other media as required. No other personal information will be knowlingly released.

I understand that there will be times that my child's baseball coach and/or parent helper may need to assist my child by positioning their body using verbal prompts, and at times, physical touch to the leg, arm or shoulders.

By signing this, I give the coaches permission to assist my child with proper baseball techniques involved with hitting, catching and throwing... using touch to the legs, arms and shoulder if needed. If, at any time, you become uncomfortable with this, please speak to your coach directly.

Rowan's Law (Concussion Safety) 2018 is a Provincial Law that came into effect in 2019 and provides requirements for sports orgnaizations like the Markdale Minor Softball Association to implement steps to prevent concussions and remove individuals from sport for a suspected/confirmed consussion until proper medical clearance is obtained.

By signing this, I acknowledge that I have fully read and commit to this Concussion Code of Conduct.

Teams without volunteer coaches may not be able to run.
