A USTA League Team Confirmation & Agreement form is required for all USTA League teams. Captains need to complete the required information listed below and sign the form. Without a complete signed form the team will not be included in the match schedule.

DEADLINE to confirm your team: This completed form is due by 5 pm on May 1, 2026.

This is a One-Weekend League (OWL): Your league coordinator will work with the host facility to secure courts and create the league schedule. Captains do NOT need to schedule their own matches for an OWL.

League Season: May 29 - 31 (OWL)

Location: Timberhill Tennis Club, Corvallis, OR.

Minimum Number of Players*: 6 players must be registered on your team by May 17

Schedule Published: in TennisLink by May 15

Sectionals: will be September 25-27, 2025 in Vancouver, WA

A team number will be sent to the Team Captain within 5 business days.
