A USTA League Team Confirmation & Agreement form is required for all USTA League teams. Captains need to complete the required information listed below and sign the form. Without a complete signed form the team will not be included in the match schedule. 

Before submitting this Team Confirmation form, please recruit the minimum number of committed players required for a team.  If you are a new captain or new to Southern Oregon and need help finding players, contact your your club's Tennis Director and/or Amanda Jordan, your USTA League Coordinator, at ajordan@pnw.usta.com

Must have facilty approval. As this is a multi-week league (and the captains will be scheduling the matches) you must notify your club's Court Scheduling Department that you will need courts for home matches.

The DEADLINE to submit this completed form to confirm you will have team is Friday, August 8.
