1st Year Player Equipment Rental Agreement

This 1st Year Player Equipment Rental Agreement (“Agreement”) is entered into between Shaler Area Youth Lacrosse Club (SAYLC) and the parent/guardian of the player listed below.

Equipment Issued
The Shaler Area Youth Lacrosse Club agrees to loan the following equipment to the above-named player for the duration of the season:
- Lacrosse helmet
- Shoulder pads
- Arm pads
- Gloves
- Any other equipment issued by SAYLC
All equipment remains the property of Shaler Area Youth Lacrosse Club.

Please Note: Stick, Mouthguard, and Athletic Cup are not provided as part of the 1st Year Player Equipment Rental Program.
Fees & Deposit
- Equipment Rental Fee: $50.00 (non-refundable) paid via cash, check, or venmo
- Equipment Deposit: $150.00 undated check, returned at end of season upon equipment return
The $150 deposit check will be held by SAYLC and returned upon timely return of all borrowed equipment in acceptable condition at the end of the season.
Care & Responsibility
The parent/guardian and player agree to:
- Use the equipment only for lacrosse-related activities
- Keep the equipment clean and in good condition
- Not alter or modify the equipment in any way
- Report any damage or safety concerns immediately
The parent/guardian is financially responsible for lost, stolen, or damaged equipment beyond normal wear and tear.
USA Lacrosse Insurance and Liability
Shaler Area Youth Lacrosse Club is a member organization of USA Lacrosse, and registered players are covered under the USA Lacrosse excess accident insurance program during sanctioned practices, games, and club activities.

The parent/guardian acknowledges and agrees that:
- Lacrosse is a contact sport and involves inherent risks of injury
- SAYLC and USA Lacrosse do not guarantee the prevention of injury
- The equipment provided is intended to reduce risk but does not eliminate it
The parent/guardian hereby releases and holds harmless Shaler Area Youth Lacrosse Club, its officers, directors, coaches, volunteers, and USA Lacrosse from any claims, demands, or liabilities arising from participation in club activities, except in cases of gross negligence or willful misconduct, to the fullest extent permitted by law.
Return of Equipment
- All equipment must be returned and cleaned as specified by SAYLC by the date designated by SAYLC.
- Equipment not returned, not cleaned as specified by SAYLC, returned late, or returned in unacceptable condition may result in partial or full forfeiture of the $150 deposit.
- Replacement costs exceeding the deposit amount may be billed to the parent/guardian.
Acknowledgment & Agreement
By signing below, I acknowledge that I have read and agree to the terms of this Equipment Rental Agreement, understand the risks associated with lacrosse participation, and accept full responsibility for the equipment issued to my child.
