Electronic Signature and Consent

By submitting this online registration application, I acknowledge and agree that my electronic submission constitutes my legal signature. I certify that all information provided is true, accurate, and complete to the best of my knowledge. I understand that my electronic signature is legally binding and has the same force and effect as a handwritten signature. I further consent to conducting this registration and receiving related communications electronically.

AGREEMENTS (Please initial)

I hereby agree to abide by the rules and regulations as established by Nitro1Sports Eagles As a parent, I understand that I must attend the parent, player and coaches' information meetings. If I am unable to attend the meetings, I understand it is my responsibility to contact a coach or team mom to retrieve the information I was not present for.

I understand the Nitro1Sports Eagles does not provide medical or hospitalization insurance whatsoever. The undersigned hereby waives any and all claims against the Nitro 1Sports Eagles or any other person affiliated with the Nitro Sports Eagles for injuries sustained while watching and/or playing games, traveling to or from organization activities. Initials: The Nitro Sports Eagles have my permission to photograph and video my child participating in practice, games, camps/clinics and I give full permission for Nitro Sports LLC to post my child/children on our team website or other Social Media Platforms.

I hereby state that my child is in good health and I understand my child may be asked to have a letter from my family physician that my child is medically cleared to participate in the track program. It is my responsibility to notify the head coach if any reason should develop that my child should not participate. Initials:

PARENTS CODE OF CONDUCT:

I understand and endorse the purpose of our program: to help boys become men of empathy and integrity who will lead, be responsible, and change the world for good. Nitro Sports is dedicated to foster and support culture by specializing in building and developing youth athletes, both on and off the field.

I support the coaches by applauding behaviors in my child and teammates that demonstrate characteristics of integrity, empathy, sacrifice, and responsibility.

I acknowledge and appreciate every athlete's growth towards maturity and efforts toward establishing stronger relationships with teammates, coaches, and themselves. I affirm my child and teammates will operate in good character, healthy sportsmanship, and other- centered behaviors are displayed. I will not only affirm athletic performance or a victory. I serve as a role model for our athletes by talking politely and acting courteously towards coaches, officials, other parents, visiting team parents, and spectators at practices, games, and meetings. I model good sportsmanship through my actions. I Acknowledge and applaud the efforts of team members and opponents. I Accept defeat graciously by congratulating the members of

the opposing team on a game/race well played. I Support the team regardless of how much or how little my child plays/races.

I encourage my child and teammates with positive statements, even when they make mistakes. At every practice they are growing physically and emotionally. At every practice they are learning moral and ethical lessons. At every practice they are developing character. I will refrain from boasting about only my child's accomplishments. When problems or questions arise, I will have my child present the problem to the head coach. This develops self-advocacy. After meeting with their coach, if the issue requires more clarity, I will contact a coach or team mom directly in a timely manner. I will refrain from speaking verbally about my issues/concerns with parents, spectators, non-organization members, etc. before I have reached out to solve my problem/concern the correct way.

I realize that no program is perfect and if any concerns arise, I will refrain from posting negative opinions about The Nitro Sports Fighting Eagles program on any social media sites and will use my coach and the board to resolve any issues properly.

I understand that I am responsible for all family and friends that attend our events and will ensure that they conduct themselves in the same manner as described above. Because I am a parent with the power, position, and platform to make a positive difference in the lives of all athletes, I commit to this code of conduct. When failing to live up to these standards, I will allow for accountability and take responsibility for my actions. The accountability/consequence may result in dismissal/removal from the Nitro Sports organization.

SOCIAL MEDIA POLICY

N1Sports Eagles recognizes the importance of social media in shaping the public's perception of Youth Sports. However, every online post is forever part of one's digital footprint. Recognizing that fact, it is expected that those both parents and players participating in the program take responsibility for their online profiles, posts, photos, videos, etc. and that everyone always represent the team and organization in a positive manner. Any unsportsmanlike, derogatory, threatening, retaliatory, stalking, harassing, bullying, criticizing, and/or knowingly false comments about teammates, coaches, parents, opponents, officials and/or staff will not be tolerated. Negative posts as described above that are brought to the attention of Nitro1Sports Board Members/CEO John Johns will be addressed as stipulated in code of conduct. Disciplinary action may include, but is not limited to verbal warnings, suspensions, and/or revocations of playing or coaching privileges. 

WAIVER AND RELEASE

Authorization for Use of Photographic Likeness

I agree to allow the Nitro1Sports to take and utilize photographic images of the registered individual(s) for the purpose of promoting and publicizing of the Program and/or events. I understand that my child's likeness may be used in such advertising. These images and videos will be used for Nitro1sports purposes only, and will not be given or sold to outside companies or individuals. If I prefer to not allow the above registered participant(s) to be photographed, I will call (253) 312-6567 to register my request immediately. I commit/agree to communicate my request prior to my athlete participating in the program.

Parent Permission

I, on behalf of my adult self or (we), parents(s) of [or legal guardian(s) for] the above participant, hereby consent to my/her/him participating in this Carroll County Department of Recreation and Parks Program. On behalf of the participant listed above, I accept the waiver of liability and assumption of the provisions of this registration form.

Media Waiver

I give my permission for my child to participate in the Nitro1Sports Program(s I also give my permission for photographs, videos, and/or voice recordings, of my child to be displayed on the official itro1Sports website and social media pages for marketing material used by the Nitro1Sports program publicly. I release Nitro1Sports or representatives or any institution transmitting, or exhibition pictures or voice of the above stated from any claims arising from such use or distribution. I have read and understand the above statements.

REFUND POLICY 

Fees are not refundable once rosters have been established and uniforms orders have been placed. A future credit may be considered for future programs if approved by the program's president Coach John Jones. We understand plans may change and this may not be the final destination of your athlete. However, we have a limited roster for the track season and once rosters are finalized, that position I reserved for the paid athlete is final. In the event that the commitment cannot be fulfilled I fully understand no refund will be issued.

WAIVER

I, the parent of my athlete, in my legal capacity as parent/legal guardian of the minor(s) named above, or as a participating adult over the age of eighteen (18), recognize and acknowledge that there are certain risks of physical injury, property damages and expenses which my child(ren) or I may sustain as a result of participating in this Program. I further agree on behalf of the minor(s) named above or myself, heirs, representatives, executors, administrators and assigns to assume all risk and agree to fully release, discharge, indemnify, hold harmless and defend Nitro Sports Eagles and its employees, volunteers, agents, and servants from any and all claims for personal injury, property damage, death or accident of any kind arising out of or in any way related to the participation in the Program, however the injury or damage occurs.

PAYMENT REMINDER:

Once we have submitted your application, it is your responsibility to submit your registration payment to coach John Jones via his Zelle account.

We are only accepting Zelle payments this year.
