    $50 non refundable deposit is due for EACH week you are registering for.

    Payment is due June 15th for July camps and July 15th, for August camps.

    Payments can be sent via e-transfer to peiintensityfoundation@gmail.com.

                    I consent to my child,   *   *, participating in the full range of activities available at PEI Intensity Summer Day Camp, including but not limited to:

                    1. Zones of freedom for free play
                    2. Swimming
                    3. Art activities
                    4. Outdoor sports
                    5. Nature walks
                    6. Interaction with farm animals


                    Media Release

                    I      *   to PEI Intensity having permission to photograph, video, and record   *   *during camp activities and to use these images and recordings to promote PEI Intensity programs in all forms, media, and manners, including but not limited to news releases, photographs, video, audio, websites, marketing, advertising, and promotion, for an indefinite period of time.

                    Liability Release

                    I understand that participation in PEI Intensity Summer Day Camp activities carries inherent risks. I hereby release and hold harmless PEI Intensity Foundation, Four Seasons Campground, and their staff, volunteers, directors, and affiliates from any liability arising from accidents, injuries, or other incidents during the program.
                    I acknowledge:

                    1. This is an outdoor camp, with children outside all day. On very hot days, water activities and shade are provided. In extreme weather, children will have access to the house.
                    2. Staff are trained in First Aid, but are not medical professionals. Children with complex medical or behavioural needs may not be able to participate.
                    3. PEI Intensity reserves the right to send children home for inappropriate behaviour, at the parent/guardian’s expense and without refund, including:
                      1. Bullying or fighting
                      2. Crude or offensive language
                      3. Disrespect to campers or staff


                    Acknowledgment & Signature

                    I have read this document in full, understand its content, and agree to its terms freely and voluntarily. By acknowledging and signing below, I am delivering an electronic signature that will have the same effect as an original manual paper signature. The electronic signature will be equally as binding as an original manual paper signature.
