Daycare Waiver and Release of Liability

By enrolling the pet(s) identified below, I represent and warrant that I am the legal owner (or authorized agent) of the pet(s) and have full authority to make all decisions regarding their care and participation.

I understand and acknowledge that participating in group play (daycare) with other pets involves inherent risks, including but not limited to injury, illness or death. While all pet play is monitored by employees of Veterinary Care of Blue Valley, P.A. and Blue Valley Operating LLC d/b/a Blue Valley Animal Hospital (collectively, the “Hospital”) to minimize risks, I recognize that scratches, punctures, torn ligaments, contagious illnesses and other injuries may occur despite reasonable supervision. I voluntarily assume all risks associated with my pet’s participation in daycare and accept full financial responsibility for any necessary treatment resulting from such injuries or illnesses. Furthermore, I release, indemnify, and hold harmless the Hospital, it’s owners, affiliates, employees, agents, and representatives from any and all claims, liabilities, damages, or expenses arising from my pet’s conduct or any injuries, illnesses, or damages sustained by my pet, myself, or others as a result of participation in daycare activities.

I affirm that my pet(s) have not displayed aggression toward people or other animals. I understand that pets showing aggression or unsafe behavior, as determined by Hospital staff, may be removed from daycare and that fees are non-refundable in such cases.

I acknowledge that the play yard may include access to water (pools, sprinklers, hoses) and outdoor surfaces that may cause my pet(s) to become wet or dirty. I accept that such amenities will not be removed to accommodate individual preferences.

I affirm that my pet(s) are current on all required vaccinations and flea prevention. I agree not to bring any pet exhibiting signs of illness (such as coughing, sneezing, vomiting, diarrhea, lethargy, or nasal/eye discharge). I understand that even with vaccination, my pet may still be exposed to contagious conditions, and I accept responsibility for any related treatment costs.

I understand that pets older than six (6) months must be spayed or neutered to participate in daycare.

I consent to the Hospital taking photos and/or videos of my pet(s) while on the premises and using such media for social media, website, and marketing purposes without compensation.

In the event of an injury or illness, I authorize the Hospital to provide medical treatment as deemed necessary and agree to be responsible for all associated costs.
