• Village Baby Showers – Registration, Consent & Agreement Form

  • Village Baby Showers offer a meaningful way to celebrate new life while supporting children and families in our community. A $100 donation covers a two-hour event and serves as the building rental; all payments for the event are considered a donation to ACTION, Inc. and are tax deductible. Your reservation includes access to bathrooms, the backyard, party rooms, and shared common spaces at the Village Center.

    Optional add-ons are available for additional fees, including cake service, table coverings, a customized photo booth, game room access, play room access, and optional cleanup services. Events must be booked within 14 days of the shower date, and refunds are only provided if your shower is more than 30 days from the confirmed event date.

    Alcohol is not permitted at the Village Center. You are welcome to bring your own food, drinks, and decorations, and tables and chairs are provided. Outside vendors—including party planners, photographers, and caterers—are allowed. While there is no access to a full kitchen, you will have access to a refrigerator. Additional event time may be added if available for your selected date and will be confirmed by Village Center staff.

    After submitting the reservation form, a member of the Village Center team will contact you to discuss your options, provide a tour of the space, and confirm your event date. If you need immediate assistance, please contact brianna@actionky.org.

    Village Baby Showers are a fundraiser benefiting programs and services at the ACTION, Inc. Village Center. By hosting your celebration with us, you are helping support families impacted by foster care through community-based resources and family-strengthening programs.
  • Section 4: What’s Included

    Each reservation includes: Use of the Village Center event space; Tables and chairs; An ACTION, Inc. staff member on site for the duration of the event; Setup and breakdown support

  • Please review the following policies:

    *An ACTION, Inc. staff member will be present during your event.
    *Decorations must be family-friendly. Confetti, glitter, nails, or adhesives that damage walls or floors are not permitted.
    *Alcohol is not permitted at the Village Center.
    *Events must end at the scheduled time to allow for cleanup.
    *Any damage beyond normal use may result in additional charges.

  • Section 7: Cleanup Expectations

  • Unless the optional cleanup service is selected, the following is expected before departure:

    ☐ All personal items, decorations, and gifts are removed
    ☐ Trash is placed in provided trash bags and bins
    ☐ Tables are cleared of food and debris
    ☐ Floors are swept and free of large spills or messes
    ☐ Furniture and room items are returned to original locations

  • Section 8: Liability & Injury Waiver (Required)

  • While ACTION, Inc. takes reasonable precautions to maintain a safe environment, participation in events may involve normal risks associated with gatherings and children’s activities.

    By signing below, I agree that:

    ACTION, Inc., its staff, and volunteers will not be held liable for minor or incidental injuries, except in cases of gross negligence or willful misconduct.
    I assume responsibility for my guests and any children attending the event.

  • Section 9: Payment & Confirmation

    After submitting this form, a team member will confirm availability and send payment instructions. Payment is required to finalize your reservation. Payments are non-refundable if changes occur within 30 days of the event. If changes occur before 30 days prior to the event, 100% of your money will be refunded.
