I fully acknowledge that with administration of medication and/or EpiPen there may be certain risks or hazards for which I will not hold NextGen FC or any of its staff, volunteers or members responsible. Participation Agreement In this agreement Next Generation Football Club (hereinafter referred to as “NextGen FC”) includes its directors, officers, employees, instructors, staff, agents, representatives, volunteers, independent contractors, subcontractors, sponsors, successors and assigns. By signing this document, I understand that I will waive or give up certain legal rights, including the right to sue for negligence, breach of contract or breach of the Occupiers’ Liability Act or claim compensation following an accident. I am aware that participation in the NextGen FC Soccer & Multi Sports camps means engaging in any of the following activities: soccer instruction, training, games; camp games; sports, speed and agility games, fitness, washroom visits; field sports, lessons, games; and, playing, resting, and eating alone and in groups, outdoors and indoors (collectively referred to as “the Activities”). These Activities involve many risks, dangers and hazards including, but not limited to: slips, trips and falls; loss of balance and control; collisions with persons, players, participants, supervisors, and referees; collisions with objects including but not limited to balls, nets, pylons, goal posts, exposed rock, trees, tree stumps; the condition of the field, grass, and areas used; changing weather conditions including lightning; accidents during participation in the Activities, choking, negligent first aid; failure to act safely or within one’s own ability or to stay within designated areas; encounters with domestic and wild animals including dogs; negligence of other persons; and risks resulting from the negligence, breach of contract, or breach of any statutory duty of care on the part of NextGen FC. I understand that negligence includes failure on the part of NextGen FC to take reasonable steps to safeguard or protect me from, or warn me of, the risks, dangers and hazards. I also understand that other risks include exposure to infectious disease contracted through viruses, bacteria, parasites, and fungi, known and unknown, which may be transmitted through direct or indirect contact, which includes COVID-19. I am also aware that the risks, dangers and hazards referred to above exist throughout and beyond the indoor and outdoor areas, fields, parking lots, sidewalks, paths, and other areas used for the Activities, and that not all hazards are marked.I freely accept and fully assume for myself and on behalf of any minor for whom I sign below all the risks, hazards, and dangers (as set out above) and the possibility of personal injury, including breaks, sprains, bruises, concussions, psychosomatic injuries and illness, aggravation of pre-existing medical conditions, death, or property loss while engaged in or as a result of my participation in the Activities.Release of Liability, Waiver of Claims and Indemnity In consideration of NextGen FC allowing me to participate in the Activities and permitting my use of the indoor and outdoor areas, fields, grass, parking, sidewalks, eating areas, resting areas, and other facilities used during the Activities (hereinafter “the Facilities”), and for other goods and valuable consideration, the receipt and sufficiently of which is acknowledged, I, an adult, or as the parent or guardian of a minor who is participating in the Activities, for myself and on behalf of that minor, hereby agree as follows:I hereby waive any and all claims that I, or anyone having a relationship to me may have now or in the future against NextGen FC arising from my use of or presence on the Facilities, or transportation to or from the Facilities.I hereby release NextGen FC from any and all liability for any loss, damage, expense, and injury and death, including any claim for contribution and indemnity, that I may suffer from participation in the Activities due to any cause whatsoever, including without limitation negligence, breach of contract and any duty of care owed to me by NextGen FC under the Occupiers’ Liability Act. I understand that negligence includes the failure on part of NextGen FC to take reasonable steps to safeguard or protect me from the risks, dangers and hazards referred to above, or to warn me of said risks dangers, and hazards.I hereby indemnify and hold harmless NextGen FC from any and all liability, including claims for contribution and indemnity, for any damage to property of or personal injury to, any third party, resulting from my, or my family member or next of kin, use of NextGen FC property, facilities and participation in the Activities.Conditions of RegistrationI/we hereby agree as follows:That the Managers or Directors of NextGen FC reserve the right to terminate the registration of any campers, when it is deemed by the Managers or Directors to be in the best interest of the child or the camp. In such an event, it is at the discretion of the Managers or Directors of NextGen FC whether to provide a proportionate refund. Your child(ren) will only be permitted to start camp once fees are paid in full prior to the start of their camp session. There are no refunds for days missed for any reason. In the event of inclement weather or other conditions requiring the camp to move locations an alternative indoor location will be provided. Indoor locations may include, but are not limited to, a school classroom or gym, community center, church or similar facility. Programming may be adjusted according to the facility's rules and regulations. NextGen FC reserves the right to change and/or update our policies and/or program offerings without notice to be in line with any changes and/or updates that the Ministry of Health may make to health and/or infectious disease guidelines. The parent/guardian of any registered camper(s) agrees to keep their camper home if they are sick and not feeling well. Campers and their guardians acknowledge that certain outdoor camp locations may not have access to permanent restroom facilities. As a result, portable toilets may be provided for the convenience of all attendees.
Participant & Family Code of Conduct: The parent/guardian acknowledges that NextGen FC maintains a strict zero-tolerance policy for any physical or verbal abuse directed at staff by parents/guardians. We kindly ask that you respect the staff members who are dedicated to caring for your children during camp. Any individual who violates this policy will be asked to withdraw their child from the camp program. Campers are expected to exhibit courteous and respectful behavior at all times. We require all campers to conduct themselves in a respectful and cooperative manner during their time at NextGen FC camp. Each camper is responsible for their own actions and behavior. Jurisdiction: This agreement and any rights, duties, and obligations as between the parties to this agreement shall be governed by and interpreted solely in accordance with the laws of Ontario and no other jurisdiction. Any litigation involving the parties to this agreement shall be brought solely within Ontario and shall be within the exclusive jurisdiction of the Courts of Ontario. Severability If any provision of this agreement is determined by a court of competent jurisdiction to be invalid, illegal, or unenforceable in any respect, such determination shall not impair or affect the validity, legality, or enforceability of the remaining provisions hereof, and each provision is hereby declared to be separate, severable and distinct. Entire Agreement In entering into this agreement, I am not relying upon any oral or written representations or statements made by NextGen FC with respect to the matters covered by this Agreement other than what is set forth in this agreement. Photo and Video Waiver and Release: I acknowledge that camp photos and video taken at soccer camp remain the sole property of such, and that they may be used in future and current print and online marketing campaigns including our website. I agree that NextGen FC can collect, use, disclose and store personal information for purposes of grouping campers, collecting/processing payments, and mail, email, or other digital and social media promotional/marketing material. Programming NextGen FC reserves the right to change or update our policies, terms and conditions, and/or programming as needed. Cancellation & Refund Policy: All camp registration fees are subject to a full refund "no questions asked" (minus any credit card processing fees) until May 18, 2026. All camp registrations must be paid in full to secure your spot at camp. Requests for changes and/or cancellations will not be accepted by telephone. All change and/or cancellation requests must be sent by email to info@nextgenfc.ca. All cancellation requests received after May 18, 2026 will be subject to a $100 cancellation fee per participant per request. Any changes to your registered weeks of camp (ie, switch to another week or another camp location) after February 16, 2026 are subject to a $15 administration fee per camper per request. No refunds will be provided once your registered camp session has started, for any reason. There are no refunds given due to inclement weather or for days that your camper(s) does not attend camp.
Acknowledgement: I have read and understand this agreement and I am aware that by signing this agreement I am waiving certain legal rights which I, the minor child on whose behalf I sign, against NextGen FC. This agreement shall be effective and binding upon my heirs, next of kin, executors, administrators, assigns and representatives, in the event of my death or incapacity. I agree to the terms and conditions herein and agree to abide by ALL policies including our policies relating to camp, before & after care / late pick up, inclement weather, anti-bullying and all other policies available at our website nextgenfc.ca or by clicking here.
