I, {parentOr}, as the Parent or Legal Guardian of the Participant, acknowledge and voluntarily consent for my child, the Participant, to participate in any “EVENT” hosted by the Arizona Tennis Classic from March 1 through March 15, 2026. Any “EVENT” specifically includes Ball Crew and/or volunteer work, as well as any other activities related to the Arizona Tennis Classic.
I understand that participation in any “EVENT” is entirely voluntary, and I am not obligated to allow my child to participate. I further acknowledge that no insurance coverage is provided by the Arizona Tennis Classic for any claims, injuries, losses, or damages arising from participation in any “EVENT.”
As the Parent or Legal Guardian of the Participant, I agree to comply with all customary rules and conditions for participation in any “EVENT” hosted by the Arizona Tennis Classic. I also agree to immediately notify an “EVENT” representative if I observe any unsafe conditions or hazards during participation.
Acknowledgment of Risks
I am aware that participation in any “EVENT” carries risks of injury, loss, or damage to the Participant, including but not limited to risks associated with running, walking, sitting, or standing during an “EVENT”; potential injuries from being struck by tennis balls or racquets; weather conditions; actions by spectators, players, officials, or other volunteers; and both known and unknown risks, including those arising from negligence, including that of the “Released Parties” (defined below). I knowingly and voluntarily assume all such risks related to the Participant’s involvement in any “EVENT.”
Release of Liability
For myself, the Participant, and our heirs, assigns, and legal representatives, I hereby release and hold harmless Arizona Tennis Charities, the Arizona Tennis Classic, the Phoenix Country Club, Phoenix Children’s Hospital, the City of Phoenix, and all current and future associated “EVENT” organizers, sponsors, participants, and their respective affiliates, officers, directors, agents, employees, contractors, volunteers, and other “EVENT” staff (collectively, the “Released Parties”) from any and all claims, demands, actions, or causes of action arising out of or related to participation in any “EVENT,” whether arising from acts of active or passive negligence, to the fullest extent permitted by law.
Medical Consent
In the event of injury or illness, I grant permission for Arizona Tennis Classic “EVENT” staff to provide basic medical treatment and to secure emergency medical care if necessary. I understand that all medical insurance coverage is the responsibility of the Parent or Legal Guardian for the Participant, and that I assume full financial responsibility for any medical expenses incurred as a result of the Participant’s participation in any “EVENT.”
Health and Safety Acknowledgment
I understand that attending any “EVENT” may present a risk of exposure to infectious or communicable diseases, including but not limited to COVID-19, influenza (flu), and other illnesses. If the Participant experiences symptoms of illness or has been exposed to a contagious disease, I agree to notify “EVENT” organizers immediately and to remove the Participant from participation.
Additionally, I agree to inform “EVENT” organizers in advance of any known food allergies or medical conditions that may require special attention to ensure the Participant’s safety.
I certify that the Participant is in good health and able to participate safely in all assigned activities related to the Arizona Tennis Classic.
Image Consent
I grant the Arizona Tennis Classic, along with all other “Released Parties,” the unrestricted right to use photographs, video recordings, or other media of the Participant captured during any “EVENT” for promotional, marketing, broadcast, or archival purposes. I waive any right to inspect or approve such media and acknowledge that no compensation will be provided for their use.
Indemnity Agreement
I agree to indemnify, defend, and hold harmless the Released Parties from any claims, damages, losses, or expenses arising from or related to the Participant’s involvement in any “EVENT.”
Legal Jurisdiction
I understand that this Release shall be governed by and interpreted in accordance with the laws of the State of Arizona. Any disputes arising under this Release shall be resolved exclusively in a court of competent jurisdiction located in Maricopa County, Arizona.
Acknowledgment of Full Understanding
I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. NO ONE HAS MADE ANY ASSURANCES OR GUARANTEES TO ME, AND I INTEND MY SIGNATURE TO SERVE AS A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
