This intake form is the first step in getting you appointed with our IMO. After this intake form, we will send you a contract to sign and we'll be able to start appointing you under FIG and our carrier appointments.
You also need to go hear to watch a video and take a quick assessment in order to start recruiting health insurance clients under us. (ONLY APPLICABLE IF YOU'RE WRITING HEALTH INSURANCE WITH US)
