This form should only be completed for Seniors attending the conference. A photo in .jpg format must also be submitted. The photo must be an official graduation image (cap/gown or formal). Casual photos, selfies, etc. are not acceptable. Please bring a copy with you to be presented at the closing banquet program. Three Teens may be submitted per form. This form is due March 1, 2026.
