Summer Camp Registration 2026

Please use this form to register for Cascadia's upcoming summer camp programs. If you have more than one student enrolled at Cascadia, please fill out this registration form for each child individually. There are enrollment limits for each class. Registration is on a first come, first serve basis. More details for each camp offering can be found here.

I, the undersigned parent or legal guardian of the Student/Child named below, acknowledge that I have voluntarily enrolled my child in Cascadia School’s 2026 Summer Programs. 

1. I acknowledge that participation in the Summer Programs may involve physical activities that carry inherent risks and dangers. I acknowledge that I have been made aware of these risks and have voluntarily allowed my child to participate.
2. I acknowledge that Cascadia School has taken reasonable steps to ensure my child's safety during summer activities, but will not be held responsible or liable for any injuries, damages, or losses my child may sustain due to their participation in the summer activities.
3. I acknowledge that Cascadia School staff may provide first aid to my child in the event of an injury or illness and may decide to call emergency services without contacting me first. I agree to release Cascadia School, its directors, agents, employees, and volunteers from any and all liability, claims, and damages arising out of or in connection with such first aid and emergency services, including but not limited to any medical treatment provided or decisions made.
4. I certify that my child has adequate medical insurance to cover any injury or damage they may suffer while participating in the Summer Programs or understand the costs of medical care without insurance.
5. I certify that my child can participate in the Summer Programs physically and mentally.
6. I acknowledge that summer program registrations are final as of May 1, 2026. No refunds or credits will be issued after this date, under any circumstances.
7. I release and hold harmless Cascadia School, its directors, agents, employees, and volunteers from any and all liability, claims, and damages arising out of or in connection with my child's participation in the Summer Programs, including any injuries, losses, or damages that may be sustained as a result of such participation.
8. I grant permission for Cascadia School to use any photographs or videos taken of my child during the Summer Programs for promotional purposes.
9. I acknowledge that I have read this release of liability statement in its entirety, that I understand its contents, and that I have had the opportunity to ask any questions that I may have about it.
